肺康复训练联合沙美特罗替卡松在稳定期慢性阻塞性肺疾病患者中的应用效果

摘要 目的:探究肺康复训练联合沙美特罗替卡松在稳定期慢性阻塞性肺疾病(以下简称“慢阻肺”)患者中的应用效果。方法:选取2020年4月-2022年4月无锡市惠山区康复医院收治的稳定期慢阻肺患者82例作为研究对象,以随机数字表法分为对照组和试验组,各41例。对照组给予沙美特罗替卡松治疗,试验组在对照组基础上给予肺康复训练。比较两组干预效果。结果:干预前,两组第1秒用力呼气容积(FEV1)、呼气峰值流速(PEF)、FEV1/用力肺活量(FVC)比较,差异无统计学意义(P>0.05);干预后,两组FEV1、PEF、FEV1/FVC高于干预前,且试验组高于对照组,差异有统计学意义(P<0.05)。干预前,两组动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO2)水平以及氧合指数(PaO_(2)/FiO_(2))比较,差异无统计学意义(P>0.05);干预后,两组PaCO_(2)低于干预前,且试验组低于对照组,两组PaO_(2)、PaO_(2)/FiO_(2)高于干预前,且试验组高于对照组,差异有统计学意义(P<0.05)。干预前,两组C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平比较,差异无统计学意义(P>0.05);干预后,两组CRP、TNF-α、IL-6水平低于干预前,且试验组低于对照组,差异有统计学意义(P<0.05)。结论:肺康复训练联合沙美特罗替卡松在稳定期慢性阻塞性肺疾病患者中的应用效果显著,可改善患者肺功能、血气指标,降低炎性因子水平。 Objective:To explore the application effect of lung rehabilitation training combined with salmeterol xinafoate and fluticasone propionate in patients with chronic obstructive pulmonary disease at stable stage.Methods:A total of 82 patients with chronic obstructive pulmonary disease at stable stage admitted to Huishan District Rehabilitation Hospital of Wuxi City from April 2020 to April 2022 were selected as the study subjects.They were divided into control group and experimental group according to random number table method,with 41 cases in each group.The control group was treated with salmeterol xinafoate and fluticasone propionate,and the experimental group was treated with lung rehabilitation training on the basis of the control group.The intervention effect was compared between the two groups.Results:Before intervention,there was no significant difference in forced expiratory volume in 1 second(FEV1),peak expiratory flow(PEF)and FEV1/forced vital capacity(FVC)between the two groups(P>0.05).After intervention,the FEV1,PEF and FEV1/FVC in the two groups were higher than those before intervention,these indicators in the experimental group were higher than those in the control group,and the difference was statistically significant(P<0.05).Before intervention,there was no significant difference in arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2))and oxygenation index(PaO_(2)/FiO_(2))between the two groups(P>0.05).After intervention,the PaCO_(2) in the two groups was lower than that before intervention,the PaCO_(2) in the experimental group was lower than that in the control group,while the PaO_(2)and PaO_(2)/FiO_(2) in the two groups were higher than those before intervention,the PaO_(2) and PaO2/FiO2 in the experimental group were higher than those in the control group,and the difference was statistically significant(P<0.05).Before intervention,there was no significant difference in the levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)between the two groups(P>0.05).After intervention,the levels of CRP,TNF-αand IL-6 in the two groups were lower than those before intervention,the levels in the experimental group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of lung rehabilitation training combined with salmeterol xinafoate and fluticasone propionate in patients with chronic obstructive pulmonary disease at stable stage has a significant effect,which can improve lung function and blood gas indexes,and reduce the level of inflammatory factors.
