血清miR-372、miR-125b、miR-592表达水平对口腔癌的临床诊断价值

摘要 目的探讨血清miR-372、miR-125b、miR-592表达水平对口腔癌的临床诊断价值。方法选择2021年7月—2023年10月安徽医科大学第一附属医院和安徽医科大学第二附属医院收治的218例口腔病患者为研究对象,按口腔病是否发生癌病变分为口腔癌组(103例)和癌前病变组(115例)两组,实时荧光定量PCR检测血清miR-372、miR-125b、miR-592;多因素Logistic回归模型分析口腔癌发生的影响因素;ROC曲线分析血清miR-372、miR-125b、miR-592对口腔癌的诊断价值。结果口腔癌组患者的血清miR-372和miR-125b水平高于癌前病变组,而血清miR-592低于癌前病变组,差异有统计学意义(P<0.05);口腔癌组患者血清血清癌胚抗原(CEA)、糖类抗原(CA125)、肿瘤坏死因子-α(TNF-α)和白细胞介素(IL-8)水平显著高于癌前病变组,差异有统计学意义(P<0.05);口腔癌患者临床分期、分化程度与miR-372表达有关(χ^(2)=7.009、8.786,P<0.05);临床分期、淋巴结转移与miR-125b、miR-592表达有关(χ^(2)=10.310、9.993、5.946、9.062,P<0.05)。多因素分析显示,miR-372(OR=1.316)、miR-125b(OR=1.258)、miR-592(OR=0.907)、CEA(OR=1.792)、CA125(OR=1.854)、TNF-α(OR=1.539)和IL-8(OR=1.445)均为口腔癌发生的影响因素(P<0.05)。ROC结果显示,血清miR-372、miR-125b、miR-592诊断口腔癌的AUC分别为0.789、0.790、0.808,三者联合诊断口腔癌的敏感度为90.29%,特异度为83.48%,AUC为0.880,显著高于miR-372(Z=2.285,P=0.022)、miR-125b(Z=2.114,P=0.035)、miR-592(Z=1.870,P=0.042)单独诊断的AUC。结论口腔癌患者血清miR-372和miR-125b水平较高,miR-592水平较低,血清miR-372、miR-125b、miR-592对口腔癌诊断价值较高。 Objective To explore the clinical diagnostic value of serum miR-372,miR-125b,and miR-592 expression levels for oral cancer.Methods A total of 218 patients suffering from oral diseases who were admitted to the First Affiliated Hospital and the Second Affiliated Hospital of Anhui Medical University from July 2021 to October 2023 were selected as the study objects.They were divided into oral cancer group and precancerous lesion group according to whether the oral disease was cancerous,and there were 103 cases of oral cancer and 115 cases of precancerous lesions.Real-time fluorescence quantitative PCR was applied to detect serum miR-372,miR-125b,and miR-592;multi factor logistic regression model was applied to analyze the influencing factors of oral cancer occurrence;ROC curve was applied to analyze the diagnostic value of serum miR-372,miR-125b,and miR-592 for oral cancer.Results The serum levels of miR-372 and miR-125b in the oral cancer group were higher than those in the precancerous group(t=9.940,8.796,P<0.05),while the serum level of miR-592 was lower than that in the precancerous group(t=8.460,P<0.05).The serum levels of carcinoembryonic antigen(CEA),carbohydrate antigen(CA125),tumor necrosis factor-α(TNF-α),and interleukin-8(IL-8)in the oral cancer group were obviously higher than those in the precancerous group(t=19.320,21.878,14.975,10.192,P<0.05).The clinical stage and differentiation degree of oral cancer patients were related to the expression of miR-372(χ^(2)=7.009,8.786,P<0.05);clinical staging and lymph node metastasis were related to the expression of miR-125b and miR-592(χ^(2)=10.310,9.993,5.946,9.062,P<0.05).Multivariate analysis showed that miR-372(OR=1.316),miR-125b(OR=1.258),miR-592(OR=0.907),CEA(OR=1.792),CA125(OR=1.854),TNF-α(OR=1.539),and IL-8(OR=1.445)were all influencing factors for the occurrence of oral cancer(P<0.05).ROC results showed that the AUC values of serum miR-372,miR-125b,and miR-592 in diagnosing oral cancer were 0.789,0.790,and 0.808,respectively,the sensitivity,specificity,and AUC of the combined diagnosis of them for oral cancer were 90.29%,83.48%,and 0.880,obviously higher than the AUC of miR-372(Z=2.285,P=0.022),miR-125b(Z=2.114,P=0.035),and miR-592(Z=1.870,P=0.042)diagnosed separately.Conclusions Serum miR-372 and miR-125b levels are high in patients with oral cancer,while miR-592 level is low.Serum miR-372,miR-125b,and miR-592 have high diagnostic value for oral cancer.
