胸腺法新联合HFNC治疗对老年重症肺炎并呼吸衰竭患者血气指标和免疫功能的影响

Effects of Thymalfasin Combined with HFNC on Blood Gas Indexes and Immune Function in Elderly Patients with Severe Pneumonia Complicated with Respiratory Failure

目的:分析探讨老年重症肺炎(SP)并呼吸衰竭(RF)患者采用胸腺法新联合经鼻高流量氧疗(HFNC)治疗对其血气指标和免疫功能的影响。方法:选取2021年7月—2023年6月宜春学院第二附属医院收治的81例老年SP并RF患者,按照乱数表法分为HFNC组(n=40)与联合组(n=41)。在常规治疗基础上,HFNC组予以HFNC治疗,联合组予以胸腺法新联合HFNC治疗。比较两组临床疗效,治疗前后血气指标、免疫功能、炎症因子,以及不良反应发生率。结果:联合组总有效率高于HFNC组,差异有统计学意义(P<0.05);治疗后两组动脉血氧分压、动脉血氧饱和度及CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)较治疗前均上升,且联合组均高于HFNC组,差异均有统计学意义(P<0.05);治疗后两组动脉血二氧化碳分压、CD8^(+)及炎症指标较治疗前均下降,且联合组均低于HFNC组,差异均有统计学意义(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:胸腺法新联合HFNC治疗老年SP并RF患者的临床效果良好,可纠正患者缺氧状态,提升免疫功能,减轻炎症反应,且安全性良好。

Objective:To analyze and explore the effects of Thymalfasin combined with high-flow nasal cannula(HFNC)on blood gas indexes and immune function in elderly patients with severe pneumonia(SP)complicated with respiratory failure(RF).Method:A total of 81 elderly patients with SP complicated with RF admitted to the Second Affiliated Hospital of Yichun University from July 2021 to June 2023 were selected and divided into HFNC group(n=40)and combined group(n=41)according to random number table method.On the basis of conventional treatment,HFNC group was treated with HFNC,and the combined group was treated with Thymalfasin combined with HFNC.The clinical efficacy,blood gas indexes,immune function,inflammatory factors and the incidence of adverse reactions were compared between the two groups.Result:The total effective rate of combined group was higher than that of HFNC group,the difference was statistically significant(P<0.05).After treatment,arterial oxygen partial pressure,arterial oxygen saturation and CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in both groups were higher than those before treatment,and those in the combined group were higher than those in the HFNC group,the differences were statistically significant(P<0.05).After treatment,the partial blood pressure of carbon dioxide,CD8^(+)and inflammation indexes in both groups were lower than those before treatment,and those in the combined group were lower than those in the HFNC group,the dif ferences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Thymalfasin combined with HFNC in the treatment of elderly patients with SP complicated with RF has good clinical effect,can correct the hypoxia state of patients,improve immune function,reduce inflammatory reaction,and has good safety.
