玻璃体切割手术治疗糖尿病视网膜病变的效果及并发症观察

Observation of Complications and Effect of Vitrectomy in the Treatment of Diabetic Retinopathy

目的:观察玻璃体切割手术治疗糖尿病视网膜病变的效果及并发症。方法:回顾性收集江西医学高等专科学校第一附属医院2021年6月—2023年3月收治的85例糖尿病视网膜病变患者的临床资料,按治疗方式的不同将85例患者分为对照组(n=42)和试验组(n=43)。对照组应用全视网膜激光光凝治疗,试验组应用玻璃体切割手术治疗。比较两组黄斑中心视网膜厚度(CRT)、最佳矫正视力(BCVA)、临床效果、并发症发生情况。结果:术后1周及1、3、6个月,试验组CRT均低于对照组(P<0.05)。相较于术前,术后1周、术后1、3、6个月,两组BCVA均逐渐下降(P<0.05),且试验组均相较于对照组低(P<0.05)。试验组总有效率相较于对照组更高,差异有统计学意义(P<0.05)。试验组并发症发生率相较于对照组更低,差异有统计学意义(P<0.05)。结论:治疗糖尿病视网膜病变,相较于应用全视网膜激光光凝治疗,应用玻璃体切割手术更能降低CRT,改善BCVA,提升效果,且有利于减少术后并发症的发生率,安全性较高,具有临床应用价值。

Objective:To observe the effect and complications of vitrectomy in the treatment of diabetic retinopathy.Method:Clinical data of 85 patients with diabetic retinopathy admitted to the First Affiliated Hospital of Jiangxi Medical College from June 2021 to March 2023 were retrospectively collected,according to different treatment methods,85 patients were classified into control group(n=42)and experimental group(n=43).The control group received panretina laser photocoagulation treatment,while the experimental group received vitrectomy.The macular central retinal thickness(CRT),best corrected visual acuity(BCVA),efficacy and the incidence of complications between two groups were compared.Result:At 1 week,1,3 and 6 months after surgery,the CRT in experimental group were lower than those in control group(P<0.05).At 1 week,1,3 and 6 months after surgery,BCVA were gradually declined in both groups compared with those before surgery(P<0.05),and those in the experimental group were lower than those in the control group(P<0.05).The total effective rate of patients in experimental group was higher compared with that in control group,the difference was statistically significant(P<0.05).The incidence of complications in the experimental group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Compared with panretinal laser photocoagulation,vitrectomy can better reduce CRT,improve BCVA,enhance efficacy,and help reduce the incidence of postoperative complications in patients with diabetic retinopathy,and it has high safety and clinical application value.
