鼻内镜手术治疗鼻窦炎鼻息肉的临床疗效分析

Analysis of the Clinical Efficacy of Endoscopic Sinus Surgery in the Treatment of Sinusitis and Nasal Polyps

目的探讨鼻内镜手术治疗鼻窦炎鼻息肉的临床疗效。方法方便选取2022年1月—2023年6月汶上县人民医院耳鼻喉科治疗的86例鼻窦炎鼻息肉患者为研究对象,按随机数表法分为两组,每组43例。对照组使用传统鼻内镜手术,观察组使用改良鼻内镜手术,对比两组的临床疗效、康复指标及并发症、症状积分及嗅觉功能评分、鼻腔通气功能及相关指标。结果观察组治疗总有效率(95.35%)明显高于对照组(74.42%),差异有统计学意义(χ^(2)=7.340,P<0.05)。观察组术创上皮化时间以及症状消失时间均短于对照组,水肿、积脓、粘连、囊泡等并发症发生率低于对照组;观察组治疗后鼻塞、水肿、流涕、头痛等症状评分低于对照组,嗅觉功能评分高于对照组;观察组鼻腔气道阻力、鼻腔鼻窦结局测试20条、嗓音障碍指数-10均低于对照组,鼻腔容积大于对照组,差异有统计学意义(P均<0.05)。结论改良鼻内镜手术治疗鼻窦炎鼻息肉的临床疗效显著,能有效缓解症状,加快鼻黏膜康复速度,改善鼻腔功能及生活质量。

Objective To explore the clinical efficacy of endoscopic sinus surgery in the treatment of sinusitis and nasal polyps.Methods A total of 86 patients with sinusitis and nasal polyps who were treated in the Department of Otolaryngology,Wenshang County People's Hospital from January 2022 to June 2023 were conveniently selected as the study objects,and divided into two groups according to the random number table method,with 43 cases in each group.The control group used traditional endoscopic sinus surgery,and the observation group used modified endoscopic sinus surgery.The clinical efficacy,rehabilitation indicators and complications,symptom scores,olfactory function scores,nasal ventilation function and related indicators were compared between the two groups.Results The treatment total effectiveness rate of the observation group was 95.35%,which was significantly higher than the 74.42%of the control group,and the difference was statistically significant(χ^(2)=7.340,P<0.05).The epithelialization time of surgical wounds and symptom disappearance time in the observation group were shorter than those in the control group,and the incidence of complications such as edema,pyema,adhesions,and vesicles was lower than that in the control group;after treatment,the symptom scores of the observation group;such as nasal congestion,edema,runny nose,and headache were lower than those of the control group,and the olfactory function score was higher than that of the control group;the observation group had lower nasal airway resistance,sino-nasal outcome test-20(SNOT-20),and voice handicap index-10(VHI-10)than the control group,the nasal cavity volume was greater than that of the control group,and the differences were statistically significant(all P<0.05).Conclusion Modified endoscopic sinus surgery has significant clinical efficacy in the treatment of sinusitis and nasal polyps,which can effectively relieve symptoms,accelerate the recovery of nasal mucosa,and improve nasal cavity function and quality of life.
