锁定钢板与股骨近端髓内钉治疗股骨粗隆下骨折的临床疗效探讨

Clinical Effect of Locking Plate and Proximal Femoral Intramedullary Nail in Treatment of Femoral Subtrochanteric Fracture

目的探究锁定钢板与股骨近端髓内钉在治疗股骨粗隆下骨折中的应用效果。方法简单随机选取如皋市第三人民医院于2021年9月—2023年6月收治的40例股骨粗隆下骨折患者作为研究对象,按照随机数表法分为对照组(20例,接受锁定钢板法治疗)与观察组(20例,接受股骨近端髓内钉治疗),对比两种手术治疗效果、手术指标、患者术后康复情况、髋关节功能情况以及不良事件发生率。结果手术治疗后,观察组治疗总有效率为95.00%,优于对照组的85.00%,差异无统计学意义(χ^(2)=1.111,P>0.05)。观察组术中出血量较少,手术时间、骨折愈合时间以及住院时间均较短,髋关节功能评分较高,并发症发生率较低,差异有统计学意义(P均<0.05)。结论锁定钢板与股骨近端髓内钉在治疗股骨粗隆下骨折中均可取得良好效果,但与锁定钢板治疗相比,股骨近端髓内钉法在缩短手术时间与康复时间指标,减少术中出血量,加速其髋关节功能改善上更具优势。

Objective To explore the effect of locking plate and proximal femoral intramedullary nail in the treatment of femoral subtrochanteric fractures.Methods A total of 40 patients with subtrochanteric fracture of femur admitted to the,Rugao Third People's Hospital from September 2021 to June 2023 were simple randomly selected as the research objects.According to the random number table method,they were divided into control group(20 cases,treated with locking plate method)and observation group(20 cases,treated with proximal femoral intramedullary nail).The therapeutic effects,surgical indexes,postoperative rehabilitation,hip joint function and incidence of adverse events were compared between the two groups.Results After surgical treatment,the total effective rate of the observation group was 95.00%,better than taht the control group(85.00%),the difference was not statistically significant(χ^(2)=1.111,P>0.05).In the observation group,the intraoperative blood loss was less,the operation time,fracture healing time and hospitalization time were shorter,the hip joint function score was higher,and the incidence of complications was lower,the differences were statistically significant(all P<0.05).Conclusion Both locking plate and proximal femoral intramedullary napping can achieve good results in the treatment of subtrochanteric fractures of femur.However,compared with locking plate treatment,proximal femoral intramedullary napping has more advantages in shortening the indexes of operation time and recovery time,reducing intraoperative blood loss,and accelerating the improvement of hip joint function.
