血浆置换在治疗视神经炎患者中的临床应用

Clinical Application of Plasma Exchange in the Treatment of Optic Neuritis

目的:分析探究血浆置换(PE)在治疗视神经炎(ON)患者中的临床应用。方法:选取2022年10月—2023年9月于赣州市人民医院进行治疗的130例ON患者,根据治疗方式的不同将其分为对照组(n=30)和观察组(n=100),对照组行大剂量甲泼尼龙冲击治疗,观察组加以PE治疗。再根据疾病类型的不同将观察组患者分为特发性ON(A组,n=34)、感染性ON(B组,n=33)及自身免疫性ON(C组,n=33)。对比各组间视力变化、PE治疗效果、不良反应。结果:治疗后,观察组视力显著优于对照组,差异有统计学意义(P<0.05);治疗后,三组视力均得到显著改善,且A组视力显著优于B组、C组(P<0.05)。观察组总有效率(89.00%)显著高于对照组(73.33%),差异有统计学意义(P<0.05);A组总有效率(100%)显著高于B组、C组(84.85%、81.82%)(P<0.05)。观察组不良反应发生率(3.00%)显著低于对照组(13.33%),差异有统计学意义(P<0.05)。结论:对ON患者采用PE治疗,能有效改善患者视力,降低不良反应发生率,具有显著的临床效果,且对于特发性ON效果更为显著。

Objective:To analyze the clinical application of plasma exchange(PE)in the treatment of optic neuritis(ON).Method:A total of 130 patients with ON who were treated in Ganzhou People's Hospital from October 2022 to September 2023 were selected,they were divided into control group(n=30)and observation group(n=100)according to different treatment methods.The control group was treated with high-dose Methylprednisolone and the observation group was treated with PE.According to the different types of diseases,the patients in the observation group were divided into idiopathic ON(group A,n=34),infectious ON(group B,n=33)and autoimmune ON(group C,n=33).The visual acuity changes,PE treatment efficacy and adverse reactions among the groups were compared.Result:After treatment,the visual acuity of observation group was significantly better than that of control group,the difference was statistically significant(P<0.05);after treatment,the visual acuity of all three groups was significantly improved,that of group A was significantly better than that of group B and group C(P<0.05).The clinical efficacy of the observation group(89.00%)was significantly higher than that of the control group(73.33%),the difference was statistically significant(P<0.05).The total effective rate in group A(100%)was significantly higher than that in group B and group C(84.85%,81.82%)(P<0.05).The incidence of adverse reactions in the observation group(3.00%)was significantly higher than that in the control group(13.33%),the difference was statistically significant(P<0.05).Conclusion:PE treatment for ON patients can effectively improve their vision and reduce the incidence of adverse reactions,which has significant clinical efficacy,especially for idiopathic ON.
