摘要
目的 评价虚拟超声模型应用于第一年住院医师的产科超声规范化培训效果,并制定基于虚拟超声模型的规范化考核方案。方法 选取2022年10月开始于北京协和医院超声医学科参加住院医师规范化培训的住院医师共14人。使用虚拟超声模型作为教具进行产科超声教学。由2名具备10年以上超声工作经验的评价者独立进行客观化评分。在培训前、后各进行一次考核并进行统计学分析。使用问卷调查方式记录住院医师自评及对虚拟超声模型教学效果的评价。结果 两次考核的评价者间一致性较强,培训前及培训后评分的组内相关系数(ICC)分别为0.900、0.785。培训后住院医师考核总分及各要素评分均得到显著提升[(83.5分, IQR:74.1~89.4)比(99.0分,IQR:95.5~99.5, P=0.001)]。培训后住院医师的晚孕检查项目扫查时间显著缩短[(21.5±11.5)分钟比(14.6±5.6)分钟, P=0.009]。住院医师认为虚拟超声模型有效提升了产科超声操作能力,并提升了产科超声检查的信心。结论 虚拟超声模型可用于第一年住院医师产科超声规范化培训及考核,为提升住院医师的临床工作胜任力提供了有效的工具方法。
Objective To evaluate the effectiveness of the virtual ultrasound model in standardized training and assessment of obstetric ultrasound skills for first-year residents.Methods A total of 14 residents who participated in the standardized training program at Peking Union Medical College Hospital starting from October 2022 were selected.The virtual ultrasound model was used as a teaching tool for obstetric ultrasound education.Two evaluators with over 10 years of ultrasound experience carried out objective assessments independently.Pre-and post-training assessments were performed and analyzed.Questionnaire surveys were used to record self-assessments by the residents and their evaluation of the effectiveness of virtual ultrasound model education.Results There were strong agreements between the evaluators in both assessments,with intraclass correlation coefficients(ICC)of 0.900 and 0.785 for pre-and post-training scores,respectively.After training,there were significant improvements in the total assessment scores and scores for each element[(83.5 points,IQR:74.1~89.4)vs(99.0points,IQR:95.5~99.5,P=0.001)].The total scan time for late pregnancy examinations by the residents decreased significantly after training[(21.5±11.5)min vs(14.6±5.6)min,P=0.009].The residents believed that the virtual ultrasound models effectively improved scanning techniques and increased confidence in obstetric examinations.Conclusion Virtual ultrasound models can be used for standardized training and assessment of obstetric ultrasound for first-year residents,providing an effective tool to enhance their clinical competency.
