宣肃止咳汤联合吸入型糖皮质激素治疗小儿支气管肺炎疗效及对血清VCAM-1、Th1/Th2细胞因子的影响

Effect of Xuansu Zhike Decoction Combined with Inhaled Glucocorticoid in the Treatment of Paediatric Bronchopneumonia and Its Effect on Serum VCAM-1 and Th1/Th2 Cytokines

目的探讨宣肃止咳汤联合吸入型糖皮质激素治疗小儿支气管肺炎的疗效及对血清血管细胞间黏附分子-1(VCAM-1)、Th1/Th2细胞因子的影响。方法选择2021年5月—2023年5月府谷县中医医院收治的80例支气管肺炎患儿作为研究对象,依据随机数表法将其分为对照组与试验组,各40例。对照组予以吸入型糖皮质激素治疗,试验组予以吸入型糖皮质激素联合宣肃止咳汤治疗。记录两组患儿临床症状消失时间,评价临床疗效,并检测治疗前后血清血管细胞间黏附分子-1(VCAM-1)、干扰素(IFN)-γ、白介素(IL-4)水平。结果与对照组比较,试验组咳嗽、喘鸣音、气促消失时间明显缩短,差异有统计学意义(P<0.05);两组发热消失时间比较,差异无统计学意义(P>0.05);试验组的总有效率高于对照组,差异有统计学意义(P<0.05);与治疗前相比,两组治疗后血清VCAM-1、IFN-γ、IL-4及IFN-γ/IL-4比值均明显改善,且试验组治疗后血清IFN-γ、IL-4及IFN-γ/IL-4比值改善明显优于对照组,差异有统计学意义(P<0.05);两组血清VCAM-1比较,差异无统计学意义(P>0.05);两组患儿均未见明显不良反应。结论宣肃止咳汤联合吸入型糖皮质激素治疗小儿支气管肺炎可有效促进症状改善,提高临床疗效,并能调节Th1/Th2细胞失衡。

Objective Objective To investigate the therapeutic effect of Xuansu Zhike Decoction combined with inhaled glucocorticoid in the treatment of bronchial pneumonia in children and the infl uence of serum vascular intercellular adhesion molecule 1(VCAM-1)and Th1/Th2 cytokines.Methods From May 2021 to May 2023,80 children of bronchopneumonia were selected.The random number table method was divided into two groups,40 cases each.The control group was treated with glucocorticoids,and the test group was treated with glucocorticoids combined with Xuanwu cough soup.The time to disappearance of clinical symptoms was recorded in both groups of children,the clinical effi cacy was evaluated,and the serum intercellular adhesion molecule-1(VCAM-1),interferon(IFN)-γ,and interleukin(IL-4)levels were detected before and after treatment.Results Compared with the control group,The duration of cough,wheezing and shortness of breath decreased significantly(P<0.05),However,the fever disappearance time of the two groups was not significant difference(P>0.05);The overall response rate of the patients in the test group was 95.00%,The overall response rate of the control patients was 77.50%,The total response rate of patients in the test group was signifi cantly higher than that in the control group(P<0.05);Compared with the control group,the serum IFN-γ,IL-4 and IFN-γ/IL-4 ratios improved signifi cantly better than those of the control group(P<0.05),and the serum VCAM-1 was not signifi cant diff erence(P>0.05);no signifi cant adverse eff ects were seen in either group.Conclusion Xuansu cough relieving decoction combined with glucocorticoid in the treatment of pediatric bronchopneumonia can eff ectively promote the improvement of symptoms,improve clinical effi cacy,and regulate the imbalance of Th1/Th2 cells.
