可弯曲负压吸引鞘联合一次性输尿管软镜治疗上尿路结石的应用

The Application of Flexible Negative Pressure Suction Sheath Combined with Disposable Flexible Ureteroscopy in the Treatment of Upper Urinary Tract Stones

目的:探讨弯曲负压吸引鞘联合一次性输尿管软镜(su-fURS)治疗上尿路结石的效果。方法:选取2022年8月~2023年8月上尿路结石患者100例。随机数字表法分为两组,包括对照组(常规鞘联合su-fURS治疗)与观察组(可弯曲负压吸引鞘联合su-fURS治疗),每组50例,对比效果。结果:对照组和观察组患者的住院时间、术中出血量相比差异不显著(P>0.05),观察组手术时间比对照组高(P<0.05),观察组住院费用、辅助体外冲击波碎石治疗率、并发症发生率比对照组少(P<0.05);术后3d,观察组结石清除率高于对照组(P<0.05),但术后1个月两组结石清除率无明显差异(P>0.05)。结论:上尿路结石应用可弯曲负压吸引鞘联合su-fURS治疗,与常规鞘联合su-fURS治疗比较,虽然会增加手术时间,导致血红蛋白下降值提升,但是却可以明显减少住院费用,避免或减少辅助体外冲击波碎石治疗,促进并发症发生率下降,同时提高结石清除率。

Objective:To evaluate the effect of curved negative pressure suction sheath combined with disposable flexible ureteroscope(su-fURS)in the treatment of upper urinary calculi.Methods:The study started in August 2022 and ended in August 2022.The 2023 was upper urinary tract stones.The patients were randomly divided into two groups:the control group(treated with conventional sheath plus su-fURS)and the observation group(treated with flexible suction sheath plus su-fURS),with 50 cases in each group,and compare the effects.Results:There was no significant difference between the two groups in the length of hospital stay and the amount of bleeding during operation(P>0.05).The operation time in the observation group was longer than that in the control group(P<0.05),compared with the control group,the patients in the observation group had lower hospitalization expenses,lower rate of auxiliary ESWL treatment and lower incidence of complications(P<0.05),SFR in the observation group was higher than that in the control group 3 days after operation(P<0.05),however,there was no significant difference in SFR between the two groups at 1 month after operation(P>0.05).Conclusion:Compared with conventional sheath combined with su-fURS treatment,although it will increase the operation time and lead to an increase in hemoglobin drop value,it can significantly reduce hospitalization costs,avoid or reduce adjuvant ESWL treatment,promote the reduction of complication rate,and improve the stone clearance.
