摘要
目的:探讨凝血真空采集管测定凝血功能精度影响多因素Logistic回归分析及干预对策。方法:选择2020年6月~2023年1月凝血功能检测患者1125例,采集血液标本并置于凝血真空采集管,采用凝血功能测定仪检测凝血功能活化部分凝血酶原时间(APTT)、纤维蛋白原(FIB)及凝血酶时间(TT)水平,根据检测精度结果分为合格组(n=1052)和不合格组(n=73)。查阅两组病例资料,对影响凝血功能检测精度可能因素进行分析,并制定干预措施。结果:1125例凝血功能测定患者,73例测定不合格,占6.45%。单因素与多因素结果显示,扎压脉带时间、溶血标本、标本中有微小凝块、试剂盒仪器灵敏度原因、标本放置温度、标本放置时间为凝血真空采集管测定凝血功能测定精度的影响因素(P<0.05)。结论:凝血真空采集管测定凝血功能不合格率占据一定比例,且受到的影响因素较多,应针对可能的影响因素制定干预对侧,提高凝血功能测定精度。
Objective:To explore the influence of multiple factors Logistic regression analysis and intervention strategies on the accuracy of coagulation function measured by coagulation vacuum collection tubes.Methods:1125 patients who underwent coagulation function testing from June 2020 to January 2023 were selected as the subjects.Blood samples were collected and placed in a coagulation vacuum collection tube.The activated partial prothrombin time(APTT),fibrinogen(FIB),and thrombin time(TT)levels of coagulation function were measured using a coagulation function analyzer.According to the detection accuracy results,they were divided into a qualified group(n=1052)and an unqualified group(n=73).Review two sets of case data,analyze possible factors that may affect the accuracy of coagulation function testing,and develop intervention measures.Results:Out of 1125 patients who underwent coagulation function tests,73 of them failed the tests,accounting for 6.45%.Single factor and multi factor results showed that the factors affecting the accuracy of coagulation function measurement using a coagulation vacuum collection tube were the time of pulse compression,hemolytic sample,presence of small clots in the sample,sensitivity of the reagent kit instrument,sample placement temperature,and sample placement time(P<0.05).Conclusion:The unqualified rate of coagulation function measured by the coagulation vacuum collection tube accounts for a certain proportion,and there are many influencing factors.Interventions should be developed based on possible influencing factors to improve the accuracy of coagulation function measurement.
