心脏彩超联合心电图对冠心病心力衰竭患者病情程度诊断的准确性评价

Evaluation of the Diagnostic Accuracy of Coronary Heart Disease and Heart Failure Using Color Doppler Ultrasound Combined with Electrocardiogram for the Severity of the Condition

目的:探讨并评价在评估冠心病心力衰竭患者疾病严重程度方面,借助心脏彩超结合心电图的准确性及价值。方法:纳入2020年6月~2023年6月本院接诊的120例冠心病心力衰竭患者,分组严格根据纽约心脏病协会(NYHA)心功能分级标准,分别设Ⅱ级组、Ⅲ级组、Ⅳ级组;均给予心电图、心脏彩超检查。结果:①心电图检查结果分析:心功能Ⅳ级的患者,QRS电压明显低于Ⅱ级、Ⅲ级患者;心功能Ⅱ级的患者,QRS时限、QTs间期短于Ⅲ级、Ⅳ级患者(P<0.05);②心脏彩超检查结果分析:Ⅱ级患者的LVESD、LVEDD、LAD以及E/A低于Ⅲ级、Ⅳ级患者(P<0.05);但Ⅳ级患者的LVEF明显低于Ⅱ级、Ⅲ级患者(P<0.05);③相关性分析发现,与心功能成正相关的因素,分别为QRS时限、QTc间期、LVESD、LVEDD、LAD、E/A。而QRS电压、LVEF与心功能之间呈现负相关关系。结论:在冠心病心力衰竭患者病情严重程度的评估中,心电图联合心脏彩超检查价值较高。

Objective:To explore the accuracy of combining electrocardiogram and echocardiography in assessing the severity of coronary heart disease and heart failure in patients.Methods:120 patients with coronary heart disease and heart failure who were admitted to our hospital from June 2020 to June 2023 were divided into class Ⅱ,Ⅲ,and Ⅳ groups according to the New York Heart Association(NYHA)cardiac function grading standards;All patients were given electrocardiogram and cardiac ultrasound examination.Results:①Analysis of electrocardiogram examination results:in patients with cardiac function grade Ⅳ,the QRS voltage was significantly lower than in patients with grade Ⅱ and Ⅲ;Patients with grade Ⅱ cardiac function had shorter QRS duration and QTs intervals than those with grade Ⅲ and Ⅳ cardiac function(P<0.05).②Analysis of cardiac ultrasound examination results:LVESD,LVEDD,LAD,and E/A of grade Ⅱ patients were lower than those of grade Ⅲ and Ⅳ patients(P<0.05);However,the LVEF of grade Ⅳ patients was significantly lower than that of grade Ⅱ and Ⅲ patients(P<0.05).③Correlation analysis found that QRS duration,QTc interval,LVESD,LVEDD,LAD,and E/A were all positively correlated with cardiac function;QRS voltage and LVEF are negatively correlated with cardiac function.Conclusion:In the assessment of the severity of coronary heart disease and heart failure in patients,the combination of electrocardiogram and cardiac ultrasound examination has a high value.
