流式荧光免疫法检测抗核抗体谱在自身免疫性疾病中的诊断价值

Diagnostic value of antinuclear antibody profile detec immunofluorescence in autoimmune diseases

目的探讨流式荧光免疫法检测抗核抗体谱在自身免疫性疾病(AID)中的诊断价值。方法选取2021年12月至2022年9月在该院就诊的269例AID患者作为AID组,另选取同期在该院体检的40例健康体检者作为正常对照组,以及51例非AID(消化系统疾病、呼吸系统疾病等)患者作为疾病对照组。采用流式荧光免疫法和免疫印迹法检测所有研究对象血清的抗核抗体谱,进行一致性评价,分析两种方法的灵敏度和特异度。结果以免疫印迹法作为金标准,流式荧光免疫法检测各组360例样本的灵敏度为92.64%,特异度为83.33%,准确度为90.00%,Kappa值为0.755;AID组中不同抗体的准确度为72.12%~98.51%,Kappa值为0.281~0.874。结论以免疫印迹法为金标准,流式荧光免疫法检测抗核抗体谱诊断AID的准确度较高,可同时检测不同抗体并精准定量,大大减少了实验室工作量,将会成为新一代的抗核抗体谱检测方法。

Objective To explore the diagnostic value of antinuclear antibody profile detected by flow immunofluorescence in autoimmune diseases(AID).Methods A total of 269 AID patients who were treated in the hospital from December 2021 to September 2022 were selected as the AID group,40 healthy people who underwent physical examination in the hospital during the same period were selected as the normal control group,and 51 patients with non-AID(digestive system diseases,respiratory diseases,etc.)were selected as the disease control group.Flow immunofluorescence and Western blot were used to detect the antinuclear antibody profiles in the serum of all subjects.The consistency was evaluated,and the sensitivity and specificity of the two methods were analyzed.Results Taking Western blot as the gold standard,the sensitivity,specificity,accuracy and Kappa value of flow fluorescence immunoassay in 360 samples of each group were 92.64%,83.33%,90.00%and 0.755,respectively.The accuracy of different antibodies in the AID group was 72.12%-98.51%,and the Kappa value was 0.281-0.874.Conclusion By western blot method as the gold standard,streaming,fluorescence immunoassay detection antinuclear antibody spectrum diagnosis AID accuracy is higher,at the same time and accurate quantitative testing different antibody,greatly reducing the workload of laboratory,will become the new generation of antinuclear antibody spectrum detection method.
