辽宁省重症肌无力患者临床特征分析

Analysis of clinical features of patients with myasthenia gravis in Liaoning province

目的分析辽宁省重症肌无力(MG)患者的临床特征。方法回顾性收集2019-09—2021-07主要由辽宁中医药大学附属医院治疗的385例MG患者的临床资料,分析性别、发病年龄、首发症状、胸腺情况等。结果与其他地区报道比,辽宁省MG患者存在一定差异,男∶女=1∶1.41,发病年龄(49.78±18.58)岁,存在两个发病高峰,75.06%的患者首发症状体现在眼部。合并胸腺瘤MG患者的发病年龄一般晚于胸腺增生(P=0.003),病理分型为B型的胸腺瘤MG患者更易累及呼吸肌(P=0.030),B1、B2型易发生肌无力危象(P=0.001),29.9%来自采矿县的患者合并胸腺瘤,而非采矿县为20.60%。MG伴其他自身免疫疾病发生率9.09%,合并白癜风2.60%。结论辽宁省MG患者与国外报道有差异,可能与地理、环境和种族有关,充分了解临床差异将有助于诊疗和预后判定。

Objective To investigate the clinical features of myasthenia gravis(MG)in Liaoning province.Methods Retrospective count and analysis were conducted to analyze 385 cases treated in Liaoning Hospital of Traditional Chinese Medicine from September 2019 to July 2021,including gender,onset age,initial symptom,thymus condition,etc.Results Compared with previous reports in other regions,MG in Liaoning province had some differences,male∶female=1∶1.41,average age of onset was(49.78±18.58)years old,and there were two peaks of onset,75.06%of the initial symptom were reflected in ocular muscles.The onset age of thymoma MG was generally later than thymoma hyperplasia(P=0.003).MG patients with thymoma pathological classification of type B was more likely to involve respiratory muscles(P=0.030).Type B1 and B2 were prone to myasthenia crisis(P=0.001),29.9%thymoma patients living in mining counties,while 20.60%of patients from non-mining counties.The incidence of MG with other autoimmune diseases was 9.09%,and with vitiligo was 2.60%.Conclusion The differences between our research and foreign reports about this study are significant,which may be related to geography,environment and ethnicity.A full and comprehensive understanding of the clinical differences will be helpful for diagnosis,treatment and prognosis.
