伏毒理论指导下miRNA靶向PI3K/Akt/mTOR通路治疗CKD动脉粥样硬化研究

摘要 慢性肾脏病(chronic kidney disease,CKD)并发心血管事件的主要病理原因是动脉粥样硬化(atherosclerosis,As),该疾病的发生与炎症密切相关。脾肾两虚是CKD As的基本病机,外邪侵犯伏于体内,聚生痰湿化毒成瘀是CKD As形成关键,而对相关炎症因子的清除与中医对伏毒的祛除微观体现相似。PI3K/Akt/mTOR信号通路能够调控相关炎症基因表达,介导炎症反应,而微小核糖核酸(micro ribonucleic acid,miRNA)可调控PI3K/Akt/mTOR信号通路抑制炎症反应干预As。中药可调控PI3K/Akt/mTOR信号通路控制炎症延缓As,且中药可通过介导miRNA调控PI3K/Akt/mTOR信号通路抑制炎症,故基于中医“正虚伏毒”理论提出“miRNA-PI3K/Akt/mTOR-正虚伏毒-CKD As”相关性假说,以期为中医药治疗CKD As提供参考思路。 The main pathological cause of chronic kidney disease(CKD)complicated with cardiovascular events is arterial Congee(As),and the occurrence of this disease is closely related to inflammation.Spleen and Kidney deficiency is the basic pathogenesis of CKD As,with external pathogens invading the body and accumulating Phlegm,Dampness,Toxins,and blood stasis.The key to the formation of CKD As is the clearance of related inflammatory factors,which is similar to the microscopic manifestation of traditional Chinese medicine in removing toxins.The PI3K/Akt/mTOR signaling pathway can regulate the expression of related inflammatory genes and mediate inflammatory responses,while microribonucleic acid(miRNA)can regulate the PI3K/Akt/mTOR signaling pathway to inhibit inflammatory responses and intervene in As.Traditional Chinese medicine can regulate the PI3K/Akt/mTOR signaling pathway to control inflammation and delay inflammation,and traditional Chinese medicine can inhibit inflammation by mediating miRNA to regulate the PI3K/Akt/mTOR signaling pathway.Therefore,based on the theory of"Zheng Qi deficiency with latent Toxin in traditional Chinese medicine,a hypothesis of"miRNA-PI3K/Akt/mTOR Zheng Qi deficiency with latent Toxin-CKD As correlation"is proposed,in order to provide some reference ideas for the treatment of CKDAs with traditional Chinese medicine.
