角膜塑形镜联合阿托品眼用凝胶对近视儿童视力及眼部综合状态的影响

Effects of orthokeratology lens combined with atropine eye gel on visual acuity and ocular comprehensive state in children with myopia

目的分析角膜塑形镜联合阿托品眼用凝胶对近视儿童视力及眼部综合状态的影响。方法纳入2019年1月至2022年1月收治的80例近视患儿为研究对象,随机将其分为常规组(40例,常规框架眼镜联合阿托品眼用凝胶)和观察组(40例,角膜塑形镜联合阿托品眼用凝胶)。比较两组的治疗效果。结果观察组的治疗总有效率显著高于常规组(P<0.05)。治疗后,观察组的水平角膜曲率显著低于常规组,最佳矫正远视力(BCDVA)、最佳矫正近视力(BCNVA)及裸眼视力显著高于常规组(P<0.05)。治疗后,观察组的等效球镜(SE)、中央角膜厚度(CCT)显著低于常规组,眼轴长度(AL)、前房深度(ACD)及晶状体厚度(LT)显著高于常规组(P<0.05)。治疗后,观察组的正相对调节量(PRA)、调节滞后量(LAG)显著低于常规组,负相对调节量(NRA)、调节幅度(AA)及调节敏感度(AS)显著高于常规组(P<0.05)。两组的并发症总发生率无显著差异(P>0.05)。结论角膜塑形镜联合阿托品眼用凝胶治疗近视儿童效果显著,可快速改善患儿近视程度,控制近视发展,且安全性高。

Objective To analyze the effects of orthokeratology lens combined with atropine eye gel on visual acuity and ocular comprehensive state in children with myopia.Methods A total of 80 children with myopia admitted from January 2019 to January 2022 were included as the study subjects and randomly divided into conventional group(40 cases,routine frame glasses combined with atropine eye gel)and observation group(40 cases,orthokeratology lens combined with atropine eye gel).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the conventional group(P<0.05).After treatment,the horizontal corneal curvature of the observation group was significantly lower than that of the conventional group,and best corrected distance visual acuity(BCDVA),best corrected near visual acuity(BCNVA)and naked eye vision were significantly higher than those of the conventional group(P<0.05).After treatment,spherical equivalent(SE)and central corneal thickness(CCT)in the observation group were significantly lower than those in the conventional group,and axial length(AL),anterior chamber depth(ACD)and lens thickness(LT)were significantly higher than those in the conventional group(P<0.05).After treatment,positive correlation regulation(PRA)and accommodative lag(LAG)in the observation group were significantly lower than those in the conventional group,and negative correlation regulation(NRA),accommodative amplitude(AA)and accommodative sensitiivity(AS)were significantly higher than those in the conventional group(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Orthokeratology lens combined with atropine eye gel in the treatment of children with myopia has an obvious effect.It can quickly improve the degree of myopia in children,control the development of myopia,and has high safety.
