结缔组织病相关肺动脉高压患者日常体力活动情况及相关因素分析

Analysis of physical activity factors related to pulmonary artery hypertension in patients with connective tissue diseases

目的探索结缔组织病相关肺动脉高压患者的日常体力活动情况,并分析其相关因素。方法采用方便抽样法选取2022年1—5月北京协和医院风湿免疫科随诊的结缔组织病相关肺动脉高压患者为研究对象,应用运动手环收集患者2周的日常体力活动数据,并对日常体力活动与临床资料之间的相关性进行分析。结果患者每日步数为6142(4724,7405)步;29.5%的患者处于久坐状态,60.3%的患者适度运动,仅10.3%的患者较为活跃。患者每日步行总距离为4.41(3.16,5.97)km、中高强度运动时间为5.57(0.61,18.48)min,每日平均睡眠时间为(7.62±0.09)h。患者日常体力活动与6 min步行距离(r=0.285,P<0.05)、B型利钠肽(r=0.351,P<0.05)及N-末端尿钠肽原(r=0.358,P<0.05)存在相关。多元Logistics回归分析显示,6 min步行实验中的疲劳指数是日常体力活动降低的独立危险因素。结论结缔组织病相关肺动脉高压患者的日常体力活动水平越高,6 min步行距离和心肺功能越好。

Objective To evaluate the physical activity levels in patients with connective tissue diseases related pulmonary artery hypertension(CTD-PAH),and to investigate its related factors.Methods From January to May 2022,78 patients with CTD-PAH were selected by convenient sampling methods.Wrist-worn accelerometers were used to collect the physical activity of the patients for two weeks,and the correlation between daily physical activity and clinical data was analyzed.Results The mean daily step count was 6142(4724,7405).29.5%of the patients were sedentary,60.3%were moderately active,and only 10.3%were relatively active.The mean total walking distance was 4.41(3.16,5.97)km,the time of moderate-to-vigorous exercise was 5.57(0.61,18.48)minutes,and the sleep time was 7.62±0.09 hours.The daily physical activity of the patients was correlated with 6MWD(r=0.285,P<0.05),BNP(r=-0.351,P<0.05),and NT-proBNP(r=-0.358,P<0.05).Multiple logistics regression analysis showed that the fatigue index in the six-minute walk test was an independent risk factor for daily physical activity reduction.Conclusion The higher level of daily physical activity could predict better 6MWD and cardiopulmonary function.
