重症监护室护士工作投入在智谋与抑郁间的中介作用

摘要 目的 探讨重症监护室(ICU)护士工作投入在智谋与抑郁间的中介效应,为制定相应干预措施提供依据。方法 采用抑郁-焦虑-压力量表中的抑郁分量表和智谋量表、工作投入量表,对河南省6所三级甲等医院1 478名ICU护士进行问卷调查。应用Pearson相关性分析探讨三者间的关系,采用AMOS 24.0构建结构方程进行作用路径分析。结果 1 478名ICU护士的智谋水平得分为(81.27±8.58)分,工作投入水平得分为(55.98±5.46)分,抑郁水平得分为(11.51±1.68)分。Pearson相关性分析显示,ICU护士工作投入与智谋呈正相关关系(r=0.227,P<0.001),抑郁与智谋、工作投入呈负相关关系(r=-0.339、-0.466,均P<0.001)。Bootstrap分析显示:智谋和工作投入对抑郁情绪均具有显著负向预测作用(β=-0.260、-0.451,均P<0.001);工作投入在智谋与抑郁情绪间起部分中介作用,中介效应值为-0.083,占总效应的24.2%。结论 ICU护士工作投入是智谋与抑郁情绪间的中介变量,护理管理者可通过提高ICU护士智谋水平,增加其工作投入度,进而降低其抑郁发生率。 Objective To investigate the mediating effect of work engagement among intensive care unit(ICU)nurses on resourcefulness and depression,and to provide a basis for the development of targeted intervention measures.Methods A questionnaire survey was conducted in among 1478 ICU nurses from six top-grade tertiary hospital in Henan Province by using the depression subscale of the depression-anxiety-stress scales(DASS),the resourceability scale and work involvement scale.Pearson correlation analysis was applied to investigate the relationships among these variables,and structural equation modeling with Amos 24.0 was utilized to analyze the pathways of influence.Results A total of 1478 ICU nurses participated in the study with mean scores of(81.27±8.58)points on the resourcefulness scale,(55.98±5.46)points on the work engagement scale,and(11.51±1.68)points on the depression scale.Pearson correlation analysis revealed a positive correlation between work engagement and resourcefulness among ICU nurses(r=0.227,P<0.001),while depression showed a significant negative correlation with both resourcefulness(r=-0.339,P<0.001),and work engagement(r=-0.466,P<0.001).Bootstrap analysis indicated that resourcefulness and work engagement have significant negative predictive effects on depressive symptoms(β=-0.260 for resourcefulness andβ=-0.451 for work engagement,all P<0.001).Work engagement partially mediated the relationship between resourcefulness and depressive mood,with a mediating effect value of-0.083,accounting for 24.2%of the total effect.Conclusions The work engagement of ICU nurses serves as a mediating variable between resourcefulness and depressive emotions.Nursing managers can improve the resourcefulness level of ICU nurses and increase their work involvement,so as to reduce the incidence of depression.
