超声对甲状腺癌合并淋巴结转移的诊断及价值评估

Ultrasound for the diagnosis and evaluation of lymph node metastasis in thyroid cancer

目的:分析甲状腺癌合并淋巴结转移患者采用超声诊断的价值。方法:选择2022年1月—2023年12月期间寿光市人民医院收治的甲状腺癌患者230例,根据患者是否合并淋巴结转移分为转移组(n=102)与未转移组(n=128)。所有患者均接受超声检查,比较不同血流信号(Ⅰ级、Ⅱ级、Ⅲ级)患者的血流动力学相关指标(动脉收缩期流速、阻力指数、动脉舒张期流速、搏动指数),并对比两组患者的相关指标(阻力指数、收缩期峰值流速)、淋巴结超声特征(淋巴结纵横向比值<2、内部回声不均、边界模糊、微钙化、血流丰富、血流信号杂乱)。结果:随着血流信号分级逐渐提升,患者的动脉收缩期流速、动脉舒张期流速逐渐上升,阻力指数、搏动指数逐渐下降,对比差异有统计学意义(P<0.01);转移组患者的血流动力学相关指标与淋巴结超声特征数据均高于未转移组,差异有统计学意义(P<0.01)。结论:超声应用于甲状腺癌合并淋巴结转移患者的诊断中准确率比较高,可根据血流动力学相关指标以及淋巴结超声征象对患者的病情予以判断。

Objective To analyze the diagnostic value of ultrasound in patients with thyroid cancer complicated with lymph node metastasis.Methods A total of 230 thyroid cancer patients admitted to our hospital from January 2022 to December 2023 were selected,and they were divided into metastatic group(n=102)and non-metastatic group(n=128)according to whether the patients had lymph node metastasis.The relevant indexes(resistance index,peak systolic flow velocity pulsatility index),lymph node ultrasound characteristics(lymph node longitudinal and transverse ratio<2,internal echo unevenness,ambiguous boundaries,microcalcifications,abundant blood flow,and chaotic blood flow signals)were compared between the two groups.Results With the gradual improvement of blood flow signal grade,the arterial systolic flow rate and arterial diastolic flow rate increased gradually,and the resistance index and pulsatility index decreased gradually,with significant difference in comparison(P<0.01).The hemodynamic related indicators and lymph node ultrasound characteristic data of patients in the metastatic group were significantly higher than those in the non-metastatic group(P<0.01).Conclusion The accuracy of ultrasound in the diagnosis of patients with thyroid cancer complicated with lymph node metastasis is relatively high,and the condition of patients can be judged according to hemodynamic indicators and lymph node ultrasound signs.
