超声引导下神经阻滞治疗带状疱疹后遗神经痛的疗效分析

Effect of ultrasound-guided nerve block for postherpetic neuralgia on NRS and PSQI scores

目的:探究超声引导下神经阻滞治疗带状疱疹后遗神经痛的近期疗效、可能作用机制以及对数字疼痛评分(NRS)、匹兹堡睡眠质量指数(PSQI)评分的影响。方法:选取某院收治的84例带状疱疹后遗神经痛患者。根据治疗方案进行分组,仅接受普瑞巴林治疗的38例患者为对照组,接受普瑞巴林联合超声引导下椎旁神经阻滞治疗的46例患者为观察组。比较两组患者的临床疗效、疼痛程度、睡眠质量、生活质量、免疫功能指标以及不良反应发生情况。结果:两组患者治疗前各指标差异均无统计学意义(P>0.05)。治疗后,相较于对照组,观察组患者临床总有效率、生活质量评分、免疫球蛋白G(IgG)、IgA以及IgM水平均明显升高(P<0.05);NRS、PSQI评分以及不良反应总发生率均明显下降(P<0.05)。结论:超声引导下神经阻滞治疗带状疱疹后遗神经痛,临床疗效显著,能有效改善患者的疼痛程度、睡眠质量以及生活质量,提高患者免疫功能,同时降低不良反应发生率,值得临床推广应用。

Objective:To explore the short-term efficacy and possible mechanism of ultrasound-guided nerve block in the treatment of postherpetic neuralgia,and its influence on digital pain score(NRS)and Pittsburgh Sleep Quality Index(PSQI).Methods:Eighty-four patients with postherpetic neuralgia who received treatment at our hospital.Grouped according to the treatment plan,38 patients who received only pregabalin treatment were the control group,and 46 patients who received pregabalin combined with ultrasound-guided paravertebral nerve block were the observation group.The clinical efficacy,pain level,sleep quality,quality of life,immune function indexes and the occurrence of adverse reactions were compared between the two groups.Results:The differences between the two groups before treatment were not significant(P>0.05).After treatment,compared with the control group,the total clinical efficiency,quality of life score,immunoglobulin G(IgG),IgA,and IgM levels of the observation group were significantly higher(P<0.05);NRS,PSQI score,and the total incidence of adverse reactions were significantly lower(P<0.05).Conclusion:Ultrasound-guided nerve block for postherpetic neuralgia has remarkable clinical efficacy,which can effectively improve the pain level,sleep quality and quality of life of the patients,and improve the immune function of the patients,and at the same time,the occurrence of adverse reactions is relatively small,which is worthy of clinical popularization and application.
