血清学指标联合检测诊断下呼吸道感染的临床意义

摘要 目的探究红细胞沉降率(ESR)、C反应蛋白(CRP)、降钙素原(PCT)单独及联合检测对下呼吸道感染的诊断价值。方法选取2020年1月至2022年6月聊城市第四人民医院收治的96例下呼吸道感染患者作为研究组,另选取同期于本院进行体检的50名健康志愿者作为对照组。两组均进行ESR、CRP与PCT检测,比较两组及研究组细菌感染者与非细菌感染者ESR、CRP与PCT水平,比较ESR、CRP与PCT单独及联合检测下呼吸道感染诊的诊断效能。结果研究组ESR、CRP、PCT水平均高于对照组,细菌感染者ESR、CRP、PCT水平均高于非细菌感染者,差异有统计学意义(P<0.05)。以临床诊断结果为金标准,ESR、CRP与PCT单独及联合检测诊断下呼吸道感染诊的准确度、灵敏度、特异度比较差异有统计学意义(P<0.05);CRP单独检测诊断准确度、特异度均高于ESR单独检测,PCT单独检测诊断准确度、灵敏度均高于ESR、CRP单独检测,特异度高于ESR单独检测,联合检测诊断准确度、灵敏度均高于PCT、CRP、ESR单独检测,且特异度均高于ESR、CRP单独检测,差异有统计学意义(P<0.05);其他检测方法准确度、灵敏度、特异度两两比较差异无统计学意义。结论ESR、CRP与PCT联合检测诊断下呼吸道感染的效果更佳。 Objective To study the value of alone and combined application of erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and procalcitonin(PCT)in the diagnosis of lower respiratory tract infection.Methods 96 patients with lower respiratory tract infection admitted to Liaocheng Fourth People's Hospital from January 2020 to June 2022 were selected as the study group,including 48 patients with bacterial infection and 48 patients with non bacterial infection.In addition,another 50 healthy volunteers who underwent physical examination in our hospital during the same period were selected as the control group.ESR,CRP and PCT were detected in both groups,the levels of ESR,CRP and PCT in bacterial infection and non-bacterial infection were compared between the two groups and the study group,the diagnostic efficacy of ESR,CRP and PCT alone and in combination in the diagnosis of lower respiratory tract infection was compared.Results The levels of ESR,CRP and PCT in the study group were higher than those in the control group,the levels of ESR,CRP and PCT in patients with bacterial infection were higher than those in patients without bacterial infection,the differences were statistically significant(P<0.05).Taking the clinical diagnosis results as the gold standard,the accuracy,sensitivity and specificity of ESR,CRP and PCT alone and combined in the diagnosis of lower respiratory tract infection were statistically significant(P<0.05);the diagnostic accuracy and specificity of CRP were higher than those of ESR,the accuracy and sensitivity of PCT were higher than those of ESR and CRP,and the specificity was higher than that of ESR,the accuracy and sensitivity of combined diagnosis were higher than those of PCT,CRP and ESR,and the specificity was higher than that of ESR and CRP,the differences were statistically significant(P<0.05);there was no significant difference in the accuracy,sensitivity and specificity of other detection methods.Conclusion Combined detection of ESR,CRP and PCT is better in the diagnosis of lower respiratory tract infection.
