基于16SrRNA基因测序对腹膜透析患者肠道微生态环境特征分析研究

摘要 目的探讨基于16SrRNA基因测序对腹膜透析患者肠道微生态环境的特征分析。方法选取2019年2—9月赣州市人民医院收治的60例接受规律性腹膜透析的患者作为观察组,另选取2019年5月于本院进行常规体检的60名受试者作为对照组。对所有受试者进行粪便提取和检测。比较两组炎症指标水平[C反应蛋白CRP)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)],分析观察组16SrRNA基因测序结果和肠道微生态检测结果。结果观察组CRP、hs-CRP、IL-6水平均高于对照组,TNF-α水平低于对照组,差异有统计学意义(P<0.05)。60例患者中,腹膜透析透出液培养检出革兰阳性菌41例,阳性率为68.3%;观察组革兰阴性菌22例(其中大肠埃希菌14例),革兰阳性菌38例(其中表皮葡萄球菌13例,头状葡萄球菌、唾液链球菌均5例)。结论16SrRNA基因测序可准确评估腹膜透析患者的肠道情况,健康人群肠道微生物的群落较复杂,但基本稳定,为肠道菌群结构和功能研究提供了参考依据。 Objective To investigate the characteristics of intestinal microenvironment in peritoneal dialysis patients based on 16SrRNA gene sequencing.Methods 60 patients who received regular peritoneal dialysis admitted to Ganzhou People's Hospital from February to September 2019 were selected as the observation group,and 60 subjects who underwent routine physical examination in our hospital in May 2019 were selected as the control group.Stool extraction and testing were performed on all subjects.The levels of inflammatory indicators[C-reactive protein CRP,hypersensitive C-reactive protein(hs-CRP),interleukin-6(IL-6),and tumor necrosis factor-α]of the two groups were compared,and the 16SrRNA gene sequencing results and intestinal micro ecological environment detection results of patients in the observation group were analyzed.Results The levels of CRP,hs-CRP and IL-6 in the observation group were higher than those in the control group,and the levels of TNF-αwere lower than those in the control group,and the differences were statistically significant(P<0.05).Among 60 patients,41 cases were detected as Gram positive bacteria in peritoneal dialysis dialysate culture,with a positive rate of 68.3%.Among the 60 patients in the observation group,there were 22 cases of Gram-negative bacteria(including 14 cases of Escherichia coli)and 38 cases of Gram-positive bacteria(including 13 cases of Staphylococcus epidermidis,5 cases of Staphylococcus capitis and 5 cases of Streptococcus salivarius).Conclusion 16SrRNA gene sequencing can accurately evaluate the intestinal status of peritoneal dialysis patients,the intestinal microbial community of healthy people is relatively complex,but basically stable,which provides a reference for the study of intestinal flora structure and function.
