防己黄芪汤联合丹参饮治疗脾肾气虚型慢性肾衰并发心衰患者肾脏血流、心功能的影响

The effect of Fangji Huangqi decoction combined with Dan Shen decoction on kidney blood flow and cardiac function in patients with chronic renal failure and heart failure of qi deficiency in spleen and kidney-type

目的探究防己黄芪汤联合丹参饮治疗脾肾气虚型慢性肾衰并发心衰患者肾脏血流、心功能的影响。方法选取2020年6月-2022年6月于本院就诊的脾肾气虚型慢性肾衰并发心衰患者82例,按照治疗方式将其分为对照组40例,联合组42例。对照组给予丹参饮治疗;联合组给予防己黄芪汤联合丹参饮治疗。评估患者中医证候积分水平;比较2组治疗前后肾功能、炎症因子、肾脏血流、心功能水平。结果治疗后,2组中医症候积分水平、肾功能水平、炎症因子水平、肾脏血流水平较治疗前均降低,且联合组较对照组下降程度更低(P<0.05);治疗后,2组心功能水平比较,左室射血分数(LVEF)水平上升,左心室收缩末期容积(LVESV)、左心室舒张末期内径(LVEDD)水平下降,且联合组优于对照组(P<0.05)。结论经防己黄芪汤联合丹参饮治疗后,脾肾气虚型慢性肾衰并发心衰患者中医证候积分水平降低,改善肾功能、改善炎症因子水平、肾脏血流水平,改善心功能水平,具有显著的优势,值得推广应用。

Objective To explore the effect of Fangji Huangqi decoction combined with Dan Shen decoction on kidney blood flow and cardiac function in patients with chronic renal failure and heart failure of qi deficiency in spleen and kidney-type.Methods 82 patients with chronic renal failure and heart failure of qi deficiency in spleen and kidney-type admitted to Fangshan Hospital of Beijing University of Chinese Medicine from June 2020 to June 2022 were selected and divided into the control group(n=40)and and the combination group(n=42)according to the treatment method.The control group was treated with Dan Shen decoction,and the combination group was treated with Fangji Huangqi decoction combined with Dan Shen decoction.Traditional Chinese medicine(TCM)syndrome scores of the patients were evaluated;The renal function,levels of inflammatory factors,renal blood flow,and cardiac function before and after treatment in both groups were compared.Results After treatment,compared with before treatment,TCM syndrome scores,renal function,levels of inflammatory factors,and renal blood flow decreased,and the degree of decrease in the combination group was lower than that in the control group(P<0.05).After treatment,the comparison of cardiac function between two groups showed that left ventricular ejection fraction(LVEF)increased,and left ventricular end systolic volume(LVESV)and left ventricular end diastolic diameter(LVEDD)decreased,and the combination group was better than the control group(P<0.05).Conclusion After the treatment of Fangji Huangqi decoction combined with Dan Shen decoction,TCM syndrome scores in patients with chronic renal failure and heart failure of qi deficiency in spleen and kidney-type decrease.This therapy can improve renal function,the levels of inflammatory factors,renal blood flow,and cardiac function of the patients.This therapy has significant advantages and is worth promoting and applying.
