新辅助化疗后3D腹腔镜胃癌根治术的安全性及近期疗效

Safety and short-term efficacy of 3-dimension laparoscopy in radical gastrectomy for gastric cancer after neoadjuvant chemotherapy

目的:分析新辅助化疗后进展期胃癌患者行3D腹腔镜胃癌根治术的安全性与近期疗效。方法:回顾分析2020年1月至2023年12月新辅助化疗后接受胃癌根治术的胃癌患者的临床资料,其中行3D腹腔镜胃癌根治术的患者纳入观察组,行传统开腹胃癌根治术者纳入对照组。对比分析两组患者临床资料、围手术期数据及术后并发症。结果:共纳入92位患者,其中观察组37例,对照组55例。观察组手术时间长于对照组,术中出血少于对照组,术后首次排气时间、首次试饮水时间、术后住院时间短于对照组,差异有统计学意义(P<0.05)。两组并发症发生率及严重程度差异无统计学意义。结论:进展期胃癌患者经新辅助化疗后行3D腹腔镜胃癌根治术具有出血量少、胃肠功能恢复快的优势,具有临床应用价值与发展前景。

Objective:This study aimed to evaluate the safety and short-term effectiveness of 3D laparoscopic radical gastrectomy in treating advanced gastric cancer following neoadjuvant chemotherapy.Methods:The data of patients who received neoadjuvant chemotherapy followed by radical gastrectomy from Jan.2020 to Dec.2023 were retrospectively collected.The patients who underwent 3D laparoscopic gastrectomy were divided into the observation group,while patients who underwent traditional open gastrectomy were in the control group.The clinical data,perioperative data,and postoperative complications of the two groups were compared.Results:A total of 92 patients were enrolled in this study,including 37 patients in observation group and 55 patients in control group.The observation group showed longer operation time,less intraoperative blood loss,shorter time to first flatus,shorter time to first oral intake,and shorter postoperative hospital stay compared with the control group(P<0.05).There was no significant difference in the incidence and severity of complications between the two groups.Conclusions:The application of 3D laparoscopic radical gastrectomy after neoadjuvant chemotherapy has the advantages of few blood loss and rapid recovery of gastrointestinal function,which has great clinical application value and development prospects.
