真武汤加减联合西药治疗心肾阳虚型心力衰竭伴频发室早30例疗效观察

Observation on the Curative Effect of Zhenwu Decoction Combined with Western Medicine on 30 Cases of Heart Failure of Heart-kidney Yang Deficiency with Frequent Ventricular Premature

目的:观察真武汤加减合胺碘酮对心肾阳虚型心力衰竭伴频发室早的疗效观察。方法:将60例患者随机分为对照组30例、观察组30例。对照组行常规优化药物治疗,另加用胺碘酮;观察组常规治疗及胺碘酮基础上,合用真武汤加减,每次1包,每日2次,共治疗2个月。比较两组服药前后中医症状积分改善情况及总有效率、左心室射血分数(LVEF)、NT-proBNP、频发室早总数改善程度。结果:两组中医症状积分均降低(P<0.05),观察组中医症状积分降低较对照组更明显(P<0.05);两组NT-proBNP值均降低(P<0.05),与对照组相比,观察组NT-proBNP值降低更优越(P<0.05);两组LVEF均提高(P<0.05),与对照组相比,观察组提高LVEF更明显(P<0.05)。两组都能降低频发室早总数(P<0.05),与对照组相比,观察组降低频发室早总数更明显(P<0.05);结论:真武汤联合胺碘酮可降低心力衰竭伴心律失常患者频发室早总数、提高LVEF及心功能分级,降低NT-proBNP、降低患者心血管事件的发生。

Objective To observe the curative effect of Zhenwu decoction plus reduced amiodarone on heart failure of heart-kidney Yang deficiency with frequent ventricular premature.Methods 60 Patients were randomly divided into control group(30 cases)and observation group(30 cases).The control group received routine optimal drug treatment,plus amiodarone;the observation group was treated with Zhenwu decoction on the basis of routine treatment and amiodarone,1 pack each time,twice a day,for a total of 2 months.The improvement of TCM symptom score,total effective rate,left ventricular ejection fraction(LVEF),NT-proBNP and the total number of frequent ventricular premature were compared between the two groups before and after taking medicine.Results Both groups can reduce the symptoms of Chinese medicine(P<0.05),the observation group reduced the scores of Chinese medicine symptoms more significantly than the control group(P<0.05);The NT-proBNP value of both groups could be reduced(P<0.05),and the NT-proBNP value of the observation group was better than that of the control group(P<0.05).LVEF was increased in both groups(P<0.05),and LVEF was increased more significantly in the observation group than in the control group(P<0.05).Both groups could reduce the total number of frequent ventricular premature occurrences(P<0.05),and the observation group was more significantly reduced than the control group(P<0.05).Conclusion Zhenwu Decoction combined with amiodarone can reduce the total number of frequent ventricular early in patients with heart failure and arrhythmia,increase LVEF and cardiac function grade,reduce NT-proBNP and reduce the occurrence of cardiovascular events.
