系统化处方点评联合药学服务对处方不合格率及不良事件的影响

Effect of systematic prescription review combined with pharmaceutical care on prescription failure rate and adverse events

目的探讨系统化处方点评联合药学服务对处方不合格率及不良事件的影响。方法选取2019年1月至2020年1月在江苏大学附属医院接受治疗的246例患者作为研究对象,其中2019年1月至6月尚未使用系统化处方点评与临床药学服务的123例患者为对照组,采用江苏大学附属医院常规的抗生素使用指导;2019年6月以后开展系统化处方点评与临床药学服务的123例患者为观察组,给予其系统化处方点评与临床药学服务。比较两组患者处方不合格率情况、不良事件发生情况、服药差错率和依从性以及两组患者满意度情况。结果观察组患者服药总依从率高于对照组,差异有统计学意义(P<0.05)。观察组服药差错总发生率低于对照组,差异有统计学意义(P<0.05)。观察组处方不合格率均低于对照组,差异有统计学意义(P<0.05)。观察组不良反应与不良事件总发生率低于对照组,差异有统计学意义(P<0.05)。观察组总满意度高于对照组,差异有统计学意义(P<0.05)。结论系统化处方点评联合药学服务能够有效提高患者依从性,降低处方不合格率以及因处方原因导致的不良事件发生率,可有效提高临床用药安全,大大提高了患者满意度,值得临床大力推广应用。

Objective To explore the effect of systematic prescription review combined with pharmaceutical care on the rate of prescription failure and adverse events.Methods A total of 246 patients who received treatment in Affiliated Hospital of Jiangsu University from January 2019 to January 2020 were selected as the study objects,among which 123 patients who had not used systematic prescription review and clinical pharmaceutical care from January to June 2019 were selected as the control group,and the routine antibiotic use guidance of Affiliated Hospital of Jiangsu University was adopted.A total of 123 patients who received systematic prescription review and clinical pharmaceutical care after June 2019 were included in the observation group,and they were given systematic prescription review and clinical pharmaceutical care.The rate of prescription failure,the occurrence of adverse events,the rate of medication error and compliance,and the satisfaction of patients in the two groups were compared.Results The total compliance of patients in observation group was higher than that in control group,the difference was statistically significant(P<0.05).The total incidence of medication errors in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The failure rate of prescription in observation group was lower than that in control group,the difference was statistically significant(P<0.05).The total incidence of adverse reactions and adverse events in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).The total satisfaction of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Systematic prescription review combined with pharmaceutical care can effectively improve patients'compliance,reduce the rate of prescription failure and the incidence of adverse events caused by prescription,effectively improve clinical drug safety,and greatly improve patients'satisfaction,which is worthy of clinical promotion and application.
