针刺联合天麻钩藤饮加减治疗眩晕肝阳上亢证疗效研究

Curative effect study on acupuncture combined with modified Tianma Gouteng Yin in treating vertigo of Ganyang Shangkang syndrome

目的:探究调督息风针刺法联合天麻钩藤饮加减治疗眩晕肝阳上亢证疗效。方法:将102例眩晕肝阳上亢证患者按照随机数字表法分为对照组、观察组,各51例。基于常规西医治疗,对照组采用天麻钩藤饮加减治疗,观察组采用调督息风针刺法联合天麻钩藤饮加减治疗,比较两组患者中医证候积分、椎基底动脉平均血流速度(MV)、血管搏动指数(PI)、血清同型半胱氨酸(Hcy)、内皮素(ET)、一氧化氮(NO)水平以及复发情况。结果:治疗后,两组中医证候积分、椎动脉与基底动脉PI、血清Hcy、ET水平与治疗前比较均降低(P<0.05),观察组降幅高于对照组(P<0.05)。两组椎动脉与基底动脉MV、血清NO水平与治疗前比较均升高(P<0.05),观察组升幅高于对照组(P<0.05)。结论:调督息风针刺法联合天麻钩藤饮加减治疗眩晕肝阳上亢证可有效调节血管内皮功能,抑制斑块生成,改善椎动脉与基底动脉血流供应,有效缓解眩晕症状。

Objective:To explore efficacy of Tiaodu Xifeng acupuncture combined with modified Tianma Gouteng Yin in treatment of patients with vertigo of Ganyang Shangkang syndrome.Methods:102 patients with vertigo of Ganyang Shangkang syndrome were classified into control group(51 cases)and observation group(51 cases)according to random number table method.On the basis of conventional western medicine treatment,the control group were treated with modified Tianma Gouteng Yin,while the observation group given Tiaodu Xifeng acupuncture combined with modified Tianma Gouteng Yin.The scores of TCM syndromes,vertebrobasilar artery mean blood flow velocity(MV),pulsatility index(PI),levels of serum homocysteine(Hcy),endothelin(ET)and nitric oxide(NO)and recurrence status were compared.Results:After treatment,the scores of TCM syndromes,PI values of vertebral artery basilar artery,and serum Hcy,ET levels in both groups were reduced compared with those before treatment(P<0.05),and the reductions in the observation group were higher than those in the control group(P<0.05).The MV values of vertebral artery and basilar artery and serum NO in the two groups were increased compared to before treatment(P<0.05),and the increases were greater in the observation group(P<0.05).Conclusion:Tiaodu Xifeng acupuncture combined with modified Tianma Gouteng Yin can effectively regulate vascular endothelial function,inhibit plaque formation,improve vertebral artery and basilar artery blood flow supply,and effectively relieve vertigo symptoms in the treatment of vertigo of Ganyang Shangkang syndrome.
