膀胱癌患者报告结局量表的编制与信效度检验

Development of a Patient-reported Outcome Scale For Bladder Cancer and Test of Reliability and Validity

目的编制基于我国国情的膀胱癌患者报告结局量表,并检验其信效度。方法以莱温的守恒模式为理论指导,通过文献回顾、半结构式访谈、认知测试、2轮德尔菲专家咨询形成初量表。采用便利抽样法选取2021年7月至11月云南省3所三级甲等医院门诊或住院部的313名膀胱癌患者进行调查,对量表进行项目分析并检验其信效度。结果最终形成的膀胱癌患者报告结局量表由4个领域、11个维度、54个条目构成。对量表的4个领域分别进行探索性因子分析,分别提取出4、2、2、3个公因子,累计方差贡献率均大于50%;量表的Cronbach’sα系数为0.950,分半信度系数为0.831;各条目的内容效度指数在0.833~1.000之间,量表的平均S-CVI为0.996,量表全体一致S-CVI为0.982。结论膀胱癌患者报告结局量表具有良好的信效度,可用于我国膀胱癌患者报告结局的评估,为临床医护人员实施治疗决策及精准护理干预提供参考依据,以期改善患者健康结局。

Objective To develop a patient-reported outcome scale for bladder cancer suitable for Chinese culture and test its reliability and validity.Methods The initial scale was formed by literature review,semistructured interview,cognitive test,and two rounds of Delphi expert consultation guided by Levin's conservation model.A total of 313 bladder cancer patients from the outpatient or inpatient departments of three tertiary care hospitals in Yunnan Province from July to November 2021 were selected by convenience sampling to investigate the scale for item analysis and reliability testing.Results The final patient-reported outcome scale for bladder cancer consisted of 4 domains,11 dimensions,and 54 items.Exploratory factor analysis was performed on the four domains of the scale,and four,two,two,and three common factors were extracted,respectively,and the cumulative variance contribution rates were greater than 50%;the Cronbach’sαcoefficient of the total scale was 0.950 and the split-half reliability coefficient was 0.831;the content validity index of each item of the scale ranged from 0.833 to 1.000,the average content validity index of the scale was 0.996,and the content validity index of allconsistent scales was 0.982. Conclusion The patient-reported outcome scale for bladder cancer has goodreliability and can be used to evaluate the patient-reported outcome of bladder cancer in China, providing areference for clinical medical staff to implement treatment decisions and precise nursing interventions to improvepatient health outcomes.
