摘要
目的:探讨血清成纤维细胞生长因子21(FGF21)和脂联素(APN)与急性心肌梗死(AMI)后急性心力衰竭(AHF)的相关性。方法:依据中国心力衰竭诊断和治疗指南诊断标准,纳入140例符合纳入和排除标准且首次诊断为AMI的患者,试验组包括急性非ST段抬高型心肌梗死(NSTEMI)组、NSTEMI合并AHF组、急性ST段抬高型心梗(STEMI)组、STEMI合并AHF组,每组35例。纳入同期在医院进行体检者70名为对照组。用ELISA法测定每组受检者血清FGF21与血清APN的含量,应用相关性分析判断两指标与AMI及AMI后AHF的相关性。结果:与未合并AHF组相比,合并AHF组血清FGF21含量升高(Z=-3.20),APN含量降低(Z=-2.05),差异均有统计学意义(均P<0.05)。与对照组相比,试验组血清FGF21含量升高(H=165.50),血清APN含量降低(H=178.70),差异均有统计学意义(均P<0.001)。相关性分析显示,血清FGF21含量与AMI后AHF发病率呈正相关(r=0.57,P<0.001),血清APN含量与AMI后AHF发病率呈负相关(r=-0.82,P<0.001)。结论:血清FGF21、APN含量与AMI后AHF发病率显著相关。
Objective:To investigate the correlation of serum fibroblast growth factor 21(FGF21)and adiponectin(APN)with acute heart failure after acute myocardial infarction(AMI).Methods:According to the diagnostic criteria of Chinese Guidelines for the Diagnosis and Treatment of Heart Failure,140 patients who met the inclusion and exclusion criteria and were diagnosed with AMI for the first time were included.The experimental groups included acute non-ST elevation myocardial infarction(NSTEMI)group,NSTEMI combined with AHF group,acute ST elevation myocardial infarction(STEMI)group,STEMI combined with AHF group.There were 35 cases in each group.A total of 70 patients who underwent physical examination in hospital during the same period were included in the control group.The contents of serum FGF21 and serum APN in each group were determined by ELISA,and the correlation between the two indexes and AMI and AHF after AMI was determined by correlation analysis.Results:Compared with the non-AHF group,the serum FGF21 content in the AHF group was increased(Z=-3.20),and the APN content was decreased(Z=-2.05),the differences were statistically significant(all P<0.05).Compared with control group,serum FGF21 content in experimental groups was increased(H=165.50),and serum APN content was decreased(H=178.70),with statistical significance(all P<0.001).Correlation analysis showed that serum FGF21 content was positively correlated with the incidence of AHF after AMI(r=0.57,P<0.001),and serum APN content was negatively correlated with the incidence of AHF after AMI(r=-0.82,P<0.001).Conclusion:Serum levels of FGF21 and APN are significantly correlated with the incidence of AHF after AMI.
