刘氏手法联合关节腔注射臭氧治疗髌骨软化症

Clinical Research of Liu's Manipulation Combined with Ozone on the Treatment of Chondromalacia Patellae

摘要 目的:观察刘氏手法联合关节腔注射臭氧治疗髌骨软化症的有效性和安全性。方法:将90例髌骨软化症患者随机分为刘氏手法组、臭氧组及刘氏手法联合臭氧组,每组30例,观察三组患者经治疗后视觉模拟量表(VAS)评分、膝关节Lysholm功能评分、髌骨匹配角及血清肿瘤坏死因子-α(TNF-α)的变化。结果:三组患者治疗后第3周和第6周VAS评分和Lysholm功能评分均优于治疗前,差异有统计学意义(P<0.05)。刘氏手法联合臭氧组在治疗后第3周和第6周,VAS评分和Lysholm功能评分均优于臭氧组和刘氏手法组,差异有统计学意义(P<0.05)。刘氏手法组在治疗后第3周和第6周,VAS评分和Lysholm功能评分均优于臭氧组,差异有统计学意义(P<0.05)。三组患者治疗后第6周髌骨匹配角及血清TNF-α均稍好于治疗前,但差异均无统计学意义(P>0.05)。结论:刘氏手法联合关节腔注射臭氧能缓解髌股关节疼痛,改善关节功能,但对髌股关节力线改变及血清炎性因子表达影响不大。 Objective:To observe the clinical efficacy of Liu's manipulation combined with intra-articular ozone injection on the treatment of chondromalacia patellae,in order to explore new idea of combining traditional Chinese and Western medicine in the treatment of patellomalacia.Methods:90 patients with chondromalacia patellae were randomly divided into ozone group,Liu's manipulation group and Liu's manipulation combined with ozone group,visual analogue scale(VAS),knee Lysholm function score,patella matching angle and serum tumor necrosis factor were measured.Results:The VAS scores and Lysholm function scores of the three groups were significantly better than those before treatment at 3 and 6 weeks after treatment(P<0.05).The VAS score and Lysholm function score in the group of Liu's manipulation combined with ozone were significantly better than those in the group of ozone and Liu's manipulation at 3 and 6 weeks after treatment(P<0.05).The VAS score and Lysholm function score in the Liu's manipulation group were significantly better than those in the ozone group at 3 and 6 weeks after treatment(P<0.05).The patella fit angle and serum tumor necrosis factor were slightly better than those before treatment,but there was no significant difference between the three groups at 6 weeks after treatment(P>0.05).Conclusion:Liu's manipulation combined with intra-articular injection of ozone can effectively relieve patellofemoral joint pain and improve joint function,but has little efficacy on the change of patellofemoral joint force line and the expression of serum inflammatory factors.
