参附注射液联合常规西药治疗急性心力衰竭患者的效果

Effects of Shenfu injection combined with conventional Western medicine in treatment of patients with acute heart failure

目的:观察参附注射液联合常规西药治疗急性心力衰竭患者的效果。方法:选取2021年1月至2023年10月该院收治的80例急性心力衰竭患者进行前瞻性研究,按照随机数字表法将其分为研究组和对照组各40例。对照组采用常规西药(单硝酸异山梨酯片+呋塞米注射液+盐酸多巴胺注射液)治疗,研究组在对照组基础上联用参附注射液治疗。比较两组临床疗效,治疗前后心功能指标[左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)]、血清心肌损伤标志物[肌酸激酶同工酶(CK-MB)、肌钙蛋白I(cTnI)、脑钠肽(BNP)]、平均动脉压、心率水平,以及不良反应发生率。结果:研究组治疗总有效率为85.00%(34/40),高于对照组的65.00%(26/40),差异有统计学意义(P<0.05);治疗后,研究组LVEF、平均动脉压均高于对照组,LVESD、LVEDD、心率及血清CK-MB、cTnI、BNP水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:参附注射液联合常规西药治疗急性心力衰竭患者效果确切,可提高心功能,改善血压、心率水平,减轻心肌损伤程度,效果优于单用常规西药治疗。

Objective:To observe effects of Shenfu injection combined with conventional Western medicine in treatment of patients with acute heart failure.Methods:A prospective study was conducted on 80 patients with acute heart failure admitted to the hospital from January 2021to October 2023.According to the random number table method,they were divided into study group and control group,40 cases in each group.The control group was treated with conventional Western medicine(Isosorbide mononitrate tablets + Furosemide injection + Dopamine hydrochloride injection),while the study group was treated with Shenfu injection on the basis of that of the control group.The clinical efficacy,the cardiac function indexes [left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF)]levels,the serum myocardial injury markers [creatine kinase isoenzyme(CK-MB),troponin I(cTnI),brain natriuretic peptide(BNP)] levels,the mean arterial pressure,the heart rate,and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the study group was 85.00%(34/40),which was higher than 65.00%(26/40) in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of LVEF and mean arterial pressure in the study group were higher than those in the control group,the levels of LVESD,LVEDD,heart rate and serum CK-MB,cTnI and BNP were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P<0.05).Conclusions:Shenfu injection combined with conventional Western medicine is effective in the treatment of the patients with acute heart failure,which can improve the cardiac function,improve the blood pressure and the heart rate,and reduce the degree of myocardial injury.Moreover,it is superior to simple conventional Western medicine treatment.
