基于知信行模式的护理干预应用于社区高血压患者护理中的效果分析

Analysis on Application Effect of Nursing Intervention Based on Knowledge,Belief and Practice Model in Nursing of Community Hypertension Patients

目的:分析基于知信行模式的护理干预应用于社区高血压患者护理中的效果。方法:选取2021年3月—2022年3月于高台县城区卫生服务中心建立档案并进行慢性病管理的高血压患者80例作为研究对象,采用摸球法随机分为对照组与观察组,各40例。对照组实施常规社区护理,观察组实施基于知信行模式的护理干预。比较两组护理效果。结果:干预3、6、12个月,两组血压达标率高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。干预后,两组治疗困扰、坚持治疗、生活方式、服药治疗评分高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:基于知信行模式的护理干预应用于社区高血压患者护理中的效果显著,能够良好地控制血压,加强患者对于血压管控的态度与信念。

Objective:To analyze the application effect of nursing intervention based on the knowledge,belief,and practice model in the nursing of hypertension patients in the community.Methods:A total of 80 hypertension patients who established records and received chronic disease management at Gaotai County Urban Health Service Center from March 2021 to March 2022 were selected as the study subjects.They were randomly divided into control group and observation group by ball touching method,with 40 cases in each group.The control group was given routine community nursing,and the observation group was given nursing intervention based on the knowledge,belief and practice model.The nursing effect was compared between the two groups.Results:After 3,6 and 12 months of intervention,the blood pressure compliance rate in the two groups was higher than that before intervention,and the rate in the observation group was higher than that in the control group,with a statistically significant difference(P<0.05).After intervention,the scores of treatment difficulties,adherence to treatment,lifestyle,and medication treatment in the two groups were higher than those before intervention,and the scores in the observation group were higher than those in the control group,with a statistically significant difference(P<0.05).Conclusion:The nursing intervention based on the knowledge,belief and practice model has a significant effect in the nursing of hypertension patients in the community,and can well control blood pressure and strengthen patients'attitudes and beliefs towards blood pressure control.
