家庭医生参与的医养结合模式应用于居家养老患者中的效果分析

Analysis on Effect of Integrated Elderly Care and Medical Service Mode Involving Family Doctors in Home-Based Care Patients

目的:分析家庭医生参与的医养结合模式应用于居家养老患者中的效果。方法:选取2021年5月—2023年4月舜玉路社区卫生服务中心管理的慢性病居家养老患者94例作为研究对象,采用随机数字表法分为研究组与对照组,各47例。对照组实施常规健康管理模式干预,研究组实施家庭医生参与的医养结合模式干预。比较两组干预效果。结果:干预后,两组疾病知识、疾病态度、应对方式、操作技巧评分高于干预前,且研究组高于对照组,差异有统计学意义(P<0.05)。干预后,两组心理功能、日常活动、生理功能、社会功能评分高于干预前,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:家庭医生参与的医养结合模式应用于居家养老患者中的效果显著,能够提高患者疾病认知水平,改善生活质量。

Objective:To analyze the effect of integrated elderly care and medical service mode involving family doctors in home-based care patients.Methods:A total of 94 home-based care patients with chronic disease who were managed by Shunyu Road Community Health Service Center from May 2021 to April 2023 were selected as the study subjects.They were divided into control group and study group according to random number table method,with 47 cases in each group.The control group received the intervention under routine health management mode,while the study group received the intervention under integrated elderly care and medical service mode involving family doctors.The intervention effects of the two groups were compared.Results:After the intervention,the scores of disease knowledge,disease attitude,coping style and operation skills in the two groups were higher than those before intervention,the scores in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).After intervention,the scores of psychological function,daily activities,physiological function and social function in the two groups were higher than those before intervention,the scores in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:The integrated elderly care and medical service mode involving family doctors has a significant effect in home-based care patients,and can improve the disease awareness level and improve the quality of life of patients.
