生物反馈电刺激联合度洛西汀治疗产后压力性尿失禁的效果

Effect of Biofeedback Electrical Stimulation Combined with Duloxetine in the Treatment of Postpartum Stress Urinary Incontinence

摘要 目的:探讨生物反馈电刺激联合度洛西汀治疗产后压力性尿失禁的效果。方法:选择2021年5月—2023年5月咸宁市妇幼保健院收治的94例产后压力性尿失禁患者为研究对象,以随机数表法将患者分为对照组(n=47)与试验组(n=47)。对照组行生物反馈电刺激治疗,试验组行生物反馈电刺激联合度洛西汀治疗。观察两组盆底功能、尿流动力学变化情况及排尿情况,比较两组治疗效果。结果:治疗后,两组Ⅰ类肌纤维肌力、Ⅱ类肌纤维肌力、阴道静息压、阴道收缩压及腹部漏尿点压(abdominal leak point pressure,ALPP)、最大尿道闭合压(maximum urethral closure pressure,MUCP)、最大尿流率(maximum flow rate,Qmax)水平高于治疗前,阴道收缩持续时间长于治疗前,且试验组优于对照组,差异有统计学意义(P<0.05)。治疗后,两组漏尿频次、排尿频次少于治疗前,最大排尿量多于治疗前,且试验组优于对照组,差异有统计学意义(P<0.05)。试验组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论:在生物反馈电刺激治疗基础上,予以产后压力性尿失禁患者度洛西汀治疗,可改善盆底功能,调节尿流动力学状态,缓解漏尿症状,提高治疗效果。 Objective:To explore the effect of biofeedback electrical stimulation combined with Duloxetine in the treatment of postpartum stress urinary incontinence.Method:A total of 94 patients with postpartum stress urinary incontinence who admitted to Xianning Maternal and Child Health Care Hospital from May 2021 to May 2023 were selected as the research objects,the patients were divided into the control group(n=47)and the experimental group(n=47)by random number table method.The control group was treated with biofeedback electrical stimulation,and the experimental group was treated with biofeedback electrical stimulation combined with Duloxetine.The pelvic floor function,urodynamic changes and urination of two groups were observed,and the therapeutic effects of two groups were compared.Result:After treatment,the levels of typeⅠmuscle fiber strength,typeⅡmuscle fiber strength,vaginal resting pressure,vaginal systolic pressure,abdominal leak point pressure(ALPP),maximum urethral closure pressure(MUCP)and maximum flow rate(Qmax)in two groups were higher than those before treatment,and the duration of vaginal contraction were longer than those before treatment,and those in the experimental group were better than those in the control group,the differences were statistically significant(P<0.05).After treatment,the frequency of urine leakage and urination in two groups were less than those before treatment,the maximum urination volume were more than those before treatment,and those in the experimental group were better than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:On the basis of biofeedback electrical stimulation therapy,the treatment of postpartum stress urinary incontinence patients with Duloxetine can improve pelvic floor function,regulate urodynamic status,alleviate urinary leakage symptoms,and improve treatment effectiveness.
