射频超声血管测量技术评估青中年2型糖尿病患者颈动脉弹性的价值

摘要 目的探讨应用射频超声血管测量技术在定量检测青中年2型糖尿病(T2DM)患者颈动脉弹性的应用价值。方法选取83例青中年T2DM患者,根据有无颈动脉斑块分为有斑块亚组(A组)和无斑块亚组(B组),同时选择40例健康者为对照组(C组)。收集三组一般资料并记录左侧颈动脉内中膜厚度(IMT)、颈动脉壁扩张性系数(DC)、顺应性系数(CC)、硬度指数(α、β)及颈动脉脉搏波传导速度(PWV)等颈动脉弹性参数。结果T2DM组左侧颈动脉DC、CC低于对照组,而IMT、α、β、PWV高于对照组差异有统计学意义(P<0.05);A组IMT较B组明显增厚(P<0.05),T2DM两亚组间PWV差异无统计学意义(P>0.05);T2DM组颈动脉PWV与IMT呈正相关(P<0.05)。结论射频超声技术可准确检测青中年T2DM患者颈动脉IMT、PWV,且PWV在颈动脉硬化早期评估颈动脉弹性减退较敏感,为临床早期预防和治疗提供一定的指导价值。 Objective To explore the application value of radiofrequency ultrasound vascular measurement technique in quantitatively detecting carotid artery elasticity in young and middle-aged patients with type 2 diabetes mellitus(T2DM).Methods A total of 83 young and middle-aged patients aged 18~60 with T2DM were selected as the case group.Based on the presence or absence of carotid artery plaques,they were divided into the plaque group(Group A)and the non-plaque group(Group B).Additionally,40 age-matched healthy individuals were selected as the control group(Group C).General clinical data were collected from the three groups,and parameters related to carotid artery elasticity including intima-media thickness(IMT),arterial Distention Coefficient(DC),compliance coefficient(CC),stiffness index(αandβ),and pulse wave velocity(PWV)of the left carotid artery were recorded.Results In the T2DM group,the left carotid artery DC and CC were lower than those in the control group,while IMT,α,β,and PWV were significantly higher than those in the normal control group,with statistical significance(P<0.05).When comparing T2DM patients with and without plaques,IMT in group A was significantly thicker than that in the group B(P<0.05),while there was no statistically significant difference in PWV between the two subgroups(P>0.05).Additionally,a positive correlation was observed between carotid artery PWV and IMT in the T2DM group(P<0.05).Conclusion Radiofrequency ultrasound technology can accurately detect carotid artery IMT and PWV in young and middle-aged patients with T2DM.Moreover,PWV is more sensitive in assessing early-stage carotid artery elasticity decline in carotid artery stiffening,providing valuable guidance for early clinical prevention and treatment.
