冠心病Hp感染患者血清Hcy和Ghrelin与MMP-9水平

Serum Hcy,Ghrelin and MMP-9 levels in coronary heart disease patients with Hp infection

目的探讨冠心病患者幽门螺杆菌(Hp)感染血清同型半胱氨酸(Hcy)和Ghrelin与基质金属蛋白酶-9(MMP-9)水平.方法回顾性选取2021年4月-2023年4月南阳市第一人民医院收治的167例冠心病患者的临床资料作为研究对象,根据是否发生Hp感染情况分为感染组(58例)和非感染组(109例).采用酶联免疫吸附实验检测血清Hcy、Ghrelin、MMP-9水平;比较两组患者血清Hcy、Ghrelin、MMP-9表达水平和冠状动脉病变程度;采用受试者工作特征(ROC)曲线分析Hcy、Ghrelin、MMP-9单独及联合检测对冠心病患者Hp感染的诊断价值.结果感染组血清Hcy、MMP-9水平均高于非感染组(P<0.05),Ghrelin水平低于非感染组(P<0.05);Hcy、Ghrelin、MMP-9单独及联合检测冠心病患者Hp感染的曲线下面积(AUC)分别为0.825、0.808、0.770和0.929;其中,联合诊断的AUC高于各项单独检测,且联合检测的敏感度和特异度为82.80%,88.10%;感染组冠状动脉病变轻度患者占比低于非感染组(P<0.05),而中度和重度患者占比高于非感染组(P<0.05).结论冠心病患者Hp感染后Hcy、MMP-9呈高表达,Ghrelin呈低表达;联合检测Hcy、Ghrelin、MMP-9有助于诊断冠心病患者Hp感染.

OBJECTIVE To investigate the levels of serum homocysteine(Hcy),Ghrelin and matrix metalloproteinase-9(MMP-9)in coronary heart disease(CHD)patients with Helicobacter pylori(Hp)infection.METHODS Clinical data of 167 patients with coronary heart disease admitted to Nanyang First People's Hospital from Apr.2021 to Apr.2023 were retrospectively analyzed,and they were divided into the infected group(58 cases)and the non-infected group(1o9 cases)according to whether the patients had Hp infection.Serum Hcy,Ghrelin and MMP-9 levels were detected by enzyme-linked immunosorbent assay.The expression levels of serum Hcy,Ghrelin,MMP-9 and the degree of coronary artery disease were compared between the two groups.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of Hcy,Ghrelin and MMP-9 alone and in combination for Hp infection in patients with coronary heart disease.RESULTS Serum Hcy and MMP-g levels in the infected group were higher than those in the non-infected group(P<o.05),while Ghrelin levels were lower than those in the non-infected group(P<0.05).The area under the curve(AUC)of Hcy,Ghrelin and MMP-9 alone and in combination for the diagnosis of Hp infection were 0.825,0.808,0.770 and 0.929,respectively.Of which,the AUC of the combined diagnosis was higher than that of the single detection,and the sensitivity and specificity of combined detection were 82.80%and 88.10%.The proportion of patients with mild coronary artery disease in the infection group was lower than that in the non-infection group(P<o.05),while the proportion of patients with moderate and severe coronary artery disease was higher than that the in non-infection group(P<0.05).CONCLUSION The expression of Hcy and MMP-9 were increased and the expression of Ghrelin was decreased in patients with CHD after Hp infection.Combined detection of Hcy,Ghrelin and MMP-9 is helpful for the diagnosis of HP infection in patients with coronary heart disease.
