复明胶囊联合康柏西普治疗糖尿病视网膜病变临床研究

Clinical Study on Fuming Capsules Combined with Conbercept for Diabetic Retinopathy

目的:观察复明胶囊联合康柏西普治疗糖尿病视网膜病变(DR)的临床疗效。方法:按随机数字表法将90例DR患者分为2组各45例。对照组给予康柏西普治疗,治疗组在对照组基础上联合复明胶囊治疗。比较2组临床疗效及不良反应,并比较2组治疗前后中医证候评分、眼部体征、最佳矫正视力及血清中细胞间黏附分子-1(ICAM-1)、色素上皮衍生因子(PEDF)、血管内皮生长因子(VEGF)水平。结果:治疗组总有效率高于对照组(P<0.05)。治疗后,2组视物模糊、目睛干涩、神疲乏力、肢体麻木等中医证候评分均降低(P<0.05),且治疗组低于对照组(P<0.05)。治疗后,2组视野灰度值、出血斑面积和中心凹视网膜厚度(CMT)均降低(P<0.05),且治疗组低于对照组(P<0.05);最佳矫正视力升高,且治疗组高于对照组(P<0.05)。治疗后,2组ICAM-1、PEDF、VEGF水平均降低(P<0.05),且治疗组低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:复明胶囊联合康柏西普能提高DR患者的临床疗效,改善临床症状,提高视力,调节VEGF的分泌,且安全性较好。

Objective:To observe the clinical effect of the therapy of Fuming Capsules combined with Conbercept on diabetic retinopathy(DR)and its effects on corrected vision.Methods:A total of 90 DR patients were divided into the control group and the treatment group by random number table method.The control group was given Conbsercept for treatment,and the treatment group was additionally treated with Fuming Capsules.The clinical effects,incidence of adverse reactions,traditional Chinese medicine syndrome scores,signs of eyes,best corrected vision and the levels of serum intercellular cell adhesion molecule-1(ICAM-1),pigment epithelium-derived factor(PEDF),and vascular endothelial growth factor(VEGF)were compared before and after treatment between the two groups.Results:The total effective rate in treatment group was higher than that in control group(P<0.05).After treatment,the above traditional Chinese medicine syndrome scores in two groups were decreased,and the scores in treatment group were lower than those in the control group(P<0.05).After treatment,the gray value of visual field,bleeding spot area and central macular thickness(CMT)in two groups were decreased,and the gray value of visual field,bleeding spot area and CMT in treatment group were lower than those in the control group(P<0.05);the best corrected vision after treatment was increased in two groups,and the best corrected vision in treatment group was higher than that in the control group(P<0.05).After treatment,the levels of ICAM-1,PEDF and VEGF in two groups were decreased(P<0.05),and the above three levels in the treatment group were lower than those in the control group(P<0.05).The incidence of adverse reactions between the two groups was no significant difference(P>0.05).Conclusion:The therapy of Fuming Capsules combined with Conbercept can enhance the clinical effect on DR patients,which can improve the clinical symptoms and vision,and regulate the secretion of VEGF with great safety.
