摘要 抗肿瘤药物是治疗恶性肿瘤的重要措施。患者的肾功能对抗肿瘤药物的选择、治疗期间的安全性及患者预后均有重要影响。所有肿瘤患者在抗肿瘤药物治疗前均应进行全面的肾功能评估,从而个体化地制定抗肿瘤方案。日本肾脏病学会、日本临床肿瘤学会、日本医学肿瘤学会和日本肾脏病药物治疗学会共同制订的《抗肿瘤药物治疗期间肾损伤管理临床实践指南(2022)》中对肿瘤患者在抗肿瘤药物治疗前肾功能的评估进行了专门的论述和建议。本文对这部分内容进行解读,以期临床对肿瘤患者中肾脏疾病的发病情况、危险因素有较全面的了解。重视肿瘤患者在抗肿瘤药物治疗前的肾功能评估,掌握正确的评估方法,有助于提高肿瘤患者与肾脏相关的治疗安全性。 Anticancer drugs play an important role in the treatment for malignant tumors.The kidney function of patients has an important impact on the choice of anticancer drugs,the safety during treatments,and the prognosis of patients.All cancer patients should undergo comprehensive kidney function assessment before using anticancer drugs,so as to formulate an individualized anticancer regimen.The Japanese Society of Nephrology,Japan Society of Clinical Oncology,Japanese Society of Medical Oncology,and Japanese Society of Nephrology and Pharmacotherapy have formulated the Clinical Practice Guidelines for Management of Kidney Injury During Anticancer Drug Therapy 2022 and made special discussions and suggestions on the evaluation of kidney function of tumor patients before anticancer drug treatment.This article interprets this part in order to made a more comprehensive understanding on the occurrence and risk factors of kidney disease in cancer patients in the clinic.Paying attention to the kidney function assessment of cancer patients before anticancer drug treatment and mastering the correct assessment methods can help improve the kidney safety of cancer patients during treatments.
