摘要
目的观察半导体激光辅助牙周基础治疗对慢性牙周炎患者的治疗疗效。方法选择2023年1月至2023年12月,慢性牙周炎患者30例,随机分为两组,每组患者15例,每位患者选取4颗指数牙,每颗牙齿探查6个位点,共60颗患牙、360个位点。对照组仅行牙周洁刮治及根面平整;试验组在上述治疗基础上辅助半导体激光照射,激光治疗光参数:波长为980 nm,功率为1.5 W,频率为10 Hz,每颗牙照射时间10 s。于治疗前、治疗后6周、治疗后12周比较两组牙周临床指标包括探诊深度、临床附着丧失、探诊出血指数;并使用菌斑显示剂检测治疗后1~5周菌斑指数改变,使用疼痛评分指数(visual analog scale,VAS)对其治疗期间疼痛程度加以记录并比较,同时比较两种治疗方式对牙龈软组织恢复时间的影响。结果治疗后6周和12周,试验组牙周临床指标、牙龈恢复时间都较对照组有显著性降低(P<0.05);VAS评分在试验组显著低于对照组(P<0.05),两种方式均能有效去除菌斑附着,且术后1周两组菌斑指数比较差异具有统计学意义(P<0.05),其他时间点比较未呈现统计学差异。结论单纯牙周基础治疗及其辅助980 nm半导体激光均能有效改善牙周临床指标,但辅助半导体激光可获得更佳的临床治疗效果。
Objective To observe the supportive effect of diode laser on basic periodontal treatment for the sake of providing other reliable methods for chronic periodontitis.Method Thirty patients diagnosed with chronic periodontitis at the outpatient clinic of Fuyang People’s Hospital were selected and divided into two groups randomly,15 patients in each group.Four index teeth were chosen from each patient,and 6 sites were explored in each tooth,resulting in a total of 60 affected teeth and 360 sites.For the teeth in the control group,only periodontal cleaning and scraping and root planing were performed.For the teeth in the experimental group,diode laser irradiation was given on the basis of the above treatments,and the parameters of the laser treatment light were set as follow,power 1.5 W,frequency 10 Hz,and irradiation time:10 s for each tooth.Periodontal clinical indexes were compared between the two groups before the treatment 6 and 12 weeks after the treatment respectively,including the depth of probing,loss of clinical attachment and bleeding index of probing.The plaque index changes that occurred 1-5 weeks after the treatment were recorded.Furthermore,the pain level during the treatment was also recorded and compared with visual analog scale(VAS),and the effect of the two treatment modalities on the gingival soft tissue recovery time was also evaluated.Results 6 and 12 weeks after the treatment,the periodontal clinical indexes and gingival recovery time in the experimental group were much less than those in the control group(P<0.05),and VAS scores were significantly lower in the experimental group than in the control group(P<0.05).Both methods were effective in removing plaque.There was a difference in the plaque indexes between the two groups 1 week after the treatment(P<0.05),but no apparent difference was presented at other time points.Conclusions Both basic periodontal treatment and auxiliary diode laser will effectively improve the periodontal clinical indicators,but the adjunctive diode laser has better clinical effect.
