清热解毒方联合地奈德乳膏对热毒炽盛证银屑病的疗效

Curative effect of Qingre Jiedu formula combined with desonide cream on psoriasis with heat toxin incandescent syndrome

目的探究清热解毒方联合地奈德乳膏对热毒炽盛证银屑病的疗效及对炎症因子和外周血T淋巴细胞亚群的影响。方法通过随机数字表法将2022年7月至2023年5月于我院进行热毒炽盛证银屑病治疗的96例患者分为试验组(n=48)和对照组(n=48)。对照组进行地奈德乳膏治疗,试验组在对照组治疗的基础上给予清热解毒方治疗,对两组银屑病面积和严重程度指数(psoriasis area and severity index,PASI)评分、中医证候评分、疗效、血清炎性因子水平、T淋巴细胞亚群水平以及不良反应进行比较。结果试验组治疗总有效率高于对照组(79.17%vs 39.58%,P<0.05)。治疗后,两组PASI、中医证候积分、白介素(interleukin,IL)-6、肿瘤坏死因子-α、神经生长因子、血管内皮生长因子、CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均较治疗前降低,且试验组低于对照组(P<0.05);两组IL-10、转化生长因子-β1及CD8^(+)均较治疗前升高,且试验组高于对照组(P<0.05)。两组不良反应发生率无统计学差异(P>0.05)。结论清热解毒方联合地奈德乳膏治疗热毒炽盛证银屑病效果较佳,能够调节炎症因子以及T淋巴细胞亚群,提高患者免疫力,且安全性良好。

Objective To investigate the therapeutic effect of Qingre Jiedu formula combined with desonide cream in the treatment of psoriasis with heat toxin incandescent syndrome and its impacts on inflammatory factors and peripheral blood T lymphocyte subsets.Methods A total of 96 patients who underwent treatment for psoriasis with heat toxin incandescent syndrome in our hospital from July 2022 to May 2023 were grouped into the experimental group(n=48)and the control group(n=48)by random number table method.The control group was treated with desonide cream,and the experimental group was treated with Qingre Jiedu formula prescription on the basis of the control group.The psoriasis area and severity index(PASI)score,traditional Chinese medicine(TCM)syndrome score,efficacy,serum inflammatory factor levels,T lymphocyte subsets levels,and adverse reactions were compared between the two groups.Results The total effective rate of the experimental group was higher than that of the control group(79.17%vs 39.58%,P<0.05).After treatment,the PASI,TCM syndrome score,interleukin(IL)-6,tumor necrosis factor-α,nerve growth factor,vascular endothelial growth factor,CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)of both group were reduced compared with those before treatment,and the experimental group was lower than the control group(P<0.05).The IL-10,transforming growth factor-β1,and CD8^(+)of both group were increased,and those of the experimental group were higher than those of the control group(P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Qingre Jiedu formula prescription combined with desonide cream has a high therapeutic effect on psoriasis with heat toxin incandescent syndrome,and can improve immunity of patients by regulating inflammatory factors and T lymphocyte subsets,with good safety.
