8例猫短暂性心肌肥厚的病例系列报告

Case Series Report of 8 Cats with Transient Myocardial Thickening

猫短暂性心肌肥厚(TMT)是一种不常见疾病,具有特殊的临床特征和疾病预后。为了分析猫TMT的临床特征、实验室检查、治疗和转归,本试验采用病例系列研究设计,收集2020—2022年诊断为猫TMT的病历资料,分析研究对象的临床特征、实验室检查、X线检查、超声心动图检查结果、治疗方案和转归。结果显示:8只猫存在舒张期左心室壁厚度增加和左心房扩张,初步诊断为“肥厚型心肌病表型”并伴有不同程度的肺水肿或胸腔积液,通过吸氧、静脉注射呋塞米和其他支持疗法治疗,症状缓解后口服呋塞米和氯吡格雷管理肥厚型心肌病。患猫在距初次超声心动图检查平均71 d(12~185 d)时复查,8只猫的左心室壁厚度和左心房大小均已恢复正常,因此,确诊为TMT。8只猫在距初次就诊平均81 d(29~187 d)时停用心脏病药物。本病例系列分析结果表明,TMT患猫的心肌增厚和左心房扩张是可逆的。本试验分析结果可为猫TMT的临床诊断和治疗提供参考。

Feline transient myocardial thickening(TMT) is a rare condition characterized by distinctive clinical features and disease prognosis.To analyze the clinical characteristics,laboratory findings,treatment,and outcomes of feline TMT,this case series study collected medical records of cats diagnosed with TMT from 2020 to 2022.The study analyzed the clinical features,laboratory tests,X-ray findings,echocardiographic results,treatment regimens,and outcomes of the subjects.The results showed that all 8 cats exhibited increased diastolic left ventricular wall thickness and left atrial dilation,initially diagnosed as "hypertrophic cardiomyopathy phenotype" with varying degrees of pulmonary edema or pleural effusion.Treatment included oxygen therapy,intravenous furosemide,and other supportive therapies,followed by oral furosemide and clopidogrel management of hypertrophic cardiomyopathy after symptom relief.Upon follow-up echocardiography,an average of 71 days(12-185 days) after the initial examination,the left ventricular wall thickness and left atrial size had returned to normal in all 8 cats,confirming the diagnosis of TMT.Cardiac medications were discontinued at an average of 81 days(29-187 days) post-initial visit.This case series analysis indicates that myocardial thickening and left atrial dilation in TMT cats are reversible.The findings of this study can provide a reference for the clinical diagnosis and treatment of feline TMT.
