复方黄柏液涂剂辅助治疗糖尿病足患者的效果及对其血清炎症因子水平的影响

Effect of Compound Cortex Phellodendri Liquid Coating on Diabetic Foot Patients and its Effect on Serum Inflammatory Factor Levels

目的:探讨复方黄柏液涂剂辅助治疗糖尿病足患者的效果及对其血清炎症因子水平的影响。方法:选取本院2020年7月—2022年10月收治的103例糖尿病足患者作为研究对象,根据随机数字表法分为手术组(n=51)和联合组(n=52),其中手术组给予Ilizarov技术胫骨横向骨搬移术治疗,联合组在手术组基础联合复方黄柏液涂剂治疗,比较两组创面愈合情况、双下肢血管造影(CTA)检查情况及血清炎症因子水平。结果:术后1周、2周、4周,两组溃疡创面均缩小,且联合组溃疡创面低于手术组(P<0.05)。术后4周,两组足背动脉管腔内径、血流量均升高,平均血流速均降低,且联合组足背动脉管腔内径、血流量高于手术组,平均血流速低于手术组(P<0.05);两组Hs-CRP、IL-6、TNF-α均降低,IL-4均升高,且联合组Hs-CRP、IL-6、TNF-α低于手术组,IL-4高于手术组(P<0.05)。结论:复方黄柏液涂剂辅助治疗能够改善糖尿病足患者足背血液循环,缩小溃疡创面,调节血清炎症因子水平。

Objective:To investigate the effect of compound Huangbai liquid coating in the adjuvant treatment of patients with diabetic foot and its effect on serum inflammatory factors.Methods:A total of 103 patients with diabetic foot admitted to our hospital from July 2020 to October 2022 were selected as the research objects.According to the random number table method,they were divided into the operation group(n=51)and the combined group(n=52).The operation group was treated with Ilizarov technique transverse tibial bone transport,and the combined group was treated with compound Huangbai liquid coating on the basis of the operation group.The wound healing,CTA examination of both lower limbs and serum inflammatory factors were compared between the two groups.Results:At 1 week,2 weeks and 4 weeks after operation,the ulcer wounds in both groups were reduced,and the ulcer wounds in the combined group were lower than those in the operation group(P<0.05).At 4 weeks after operation,the inner diameter and blood flow of dorsalis pedis artery in the two groups increased,and the average blood flow rate decreased,while the inner diameter and blood flow of dorsalis pedis artery in the combined group were higher than those in the operation group,and the average blood flow rate was lower than that in the operation group(P<0.05).The levels of Hs-CRP,IL-6 and TNF-αin the two groups decreased,and IL-4 increased,while the levels of Hs-CRP,IL-6 and TNF-αin the combined group were lower than those in the operation group,and IL-4 was higher than that in the operation group(P<0.05).Conclusion:The adjuvant treatment of compound Huangbai liquid coating can improve the blood circulation of dorsal foot in patients with diabetic foot,reduce the ulcer wound,and regulate the level of serum inflammatory factors.
