2018—2022年海南某医院住院部第二类精神药品使用情况分析

Analysis on the use of classⅡpsychotropic drugs in theinpatient department of a hospital in Hainan from 2018 to 2022

目的对海南某医院住院部第二类精神药品的使用情况进行分析,促进临床合理用药。方法提取2018—2022年海南某医院住院部第二类精神药品的使用信息,对其销售金额、用药频度、日均药费、B/A值等数据进行分析。结果2018—2021年该院住院部第二类精神药品用量呈逐年减少趋势,而2022年使用量达到新高。第二类精神药品销售金额浮动较大,2022年的金额占比最高。销售金额排名前三位的是酒石酸布托啡诺注射液、地佐辛注射液及咪达唑仑注射液。历年的DDDs排序变化较小,排序第一的为艾司唑仑,其次为氨酚羟考酮。DDDc保持平稳,地佐辛注射液、酒石酸布托啡诺注射液和咪达唑仑注射液始终位于前三位,其中地佐辛注射液连续五年居第1位。艾司唑仑片和酒石酸唑吡坦片的B/A大于1.00,说明该药价格低廉,患者接受度较高。酒石酸布托啡诺注射液和地佐辛注射液的B/A<1.00,说明该药价格偏贵,患者接受度低。其余药品的B/A接近1.00。结论该院住院部第二类精神药品的临床应用基本合理,继续加强监督和管理,促进科学、有效、经济及合理地使用第二类精神药品。

Objective The use of classⅡpsychotropic drugs in the inpatient department of a hospital in Hainan was analyzed to promote clinical rational drug use.Methods The use information of classⅡpsychotropic drugs in the inpatient department of the hospital from 2018 to 2022 was extracted,and the data of the use amount,sales amount,DDDs,DDDc,and B/A value were analyzed.Results From 2018 to 2021,the consumption of classⅡpsychotropic drugs decreased year by year,but in 2022 reached a new high.The sales amount of classⅡpsychotropic drugs fluctuates greatly.The largest proportion of total sale amount is in 2022.The top three in terms of sales were butorphanol tartrate injection,dezocine injection and midazolam injection.The order of DDDs changed little,the first was estazolam,the second was oxycodone and acetaminophen.The DDDc remained stable.Dezocine injection,butorphanol tartrate injection and midazolam injection remaining in the first three places.Dezocine injection was in the first place for five consecutive years.The B/A of estazolam tablets and zolpidem tartrate tablets were higher than 1.00,which indicated that the drug was cheap and acceptable to patients.The B/A of butorphanol tartrate injection and dezocine injection were less than 1.00.It indicated that the drug was expensive and low acceptance by patients.The B/A ratio for the rest classⅡpsychotropic drugs was close to 1.00.Conclusion The clinical application of classⅡpsychotropic drugs in the inpatient department of this hospital is basically reasonable,and supervision and management should be strengthened to promote the scientific,effective,economic and rational use of classⅡpsychotropic drugs.
