甲状腺功能检测在2型糖尿病并发正常甲状腺病态综合征患者诊断及疗效评价中的作用

The role of thyroid function indicators in the diagnosis and efficacy evaluation of euthyroid sick syndrome patients with type 2 diabetes

目的 分析甲状腺功能3项检测在2型糖尿病并发正常甲状腺病态综合征(ESS)患者诊断及疗效评价中的作用。方法 选择2020年11至2023年11月收治的68例2型糖尿病并发ESS患者作为研究组,选择同期收治的糖尿病非ESS患者68例作为对照组。比较治疗前后2组患者游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺素(TSH)水平;比较2组患者甘油三酯、糖化血红蛋白、血糖水平。结果对照组患者血清FT3、FT4水平高于研究组(P<0.05)。2组患者血清TSH、甘油三酯水平比较差异均无统计学意义(P>0.05)。糖化血红蛋白、血糖水平在2组间比较,研究组较高(P<0.05)。治疗后研究组患者血清FT3、FT4水平均升高(P<0.05)。血清TSH水平无显著变化(P>0.05)。结论 2型糖尿病并发ESS患者存在甲状腺功能异常,而甲状腺功能3项水平异常程度与患者全身状态具有相关性,对患者的甲状腺功能3项进行检测,有利于评估患者疾病转归情况,在疗效评价中发挥作用。

Objective To analyze the role of three indicators of thyroid function in the diagnosis and effi-cacy evaluation of patients with diabetes complicated with euthyroid sick syndrome(ESS).Methods Sixty-eight patients with diabetes complicated with ESS were selected as the study group,and 68 patients with diabetes with-out ESS during the same period were selected as the control group.Patients in both groups were admitted from November 2020 to November 2023.FT3,FT4 and TSH levels were compared between the two groups.The levels of triglyceride,glycosylated hemoglobin and blood glucose were compared between the two groups.The levels of FT3,FT4 and TSH in patients were compared between the two groups after treatment.Results The levels of serum FT3 and FT4 in control group were higher than those in study group(P<0.05).There was no significant dif-ference in serum TSH level between the two groups(P>0.05).There was no significant difference in triglyceride between the two groups(P>0.05).The levels of glycosylated hemoglobin and blood glucose were higher in the study group(P<0.05).After treatment,serum FT3 and FT4 levels in the study group were increased(P<0.05).There were no significant changes in serum TSH level(P>0.05).Conclusion Thyroid dysfunction exists in pa-tients with diabetes complicated with ESS,and the abnormal levels of three indicators of thyroid function are cor-related with the overall condition of patients.The detection of three indicators of thyroid function in patients is conducive to evaluating the disease outcome of patients,and plays a role in the evaluation of curative effect.
