不同性别2型糖尿病患者糖化血红蛋白A1c和尿微量白蛋白/尿肌酐与血清白蛋白、尿酸、肌酐及甘油三脂的相关性研究

摘要 目的分析不同性别2型糖尿病(T2DM)患者糖化血红蛋白A1c(HbA1c)、尿微量白蛋白/尿肌酐(ACR)与血清白蛋白(ALB)、血清尿酸(SUA)、血清肌酐(SCr)、甘油三酯(TG)的相关性。方法回顾性分析2020年7月至2021年7月微山县人民医院内分泌科收治住院的597例2型糖尿病患者的临床资料,所有患者均进行血液样本、尿液样本检测。比较不同性别T2DM患者临床资料,采用Person相关分析不同性别T2DM患者HbA1c、ACR与ALB、SUA、SCr、TG的相关性。结果男性T2DM患者SUA、SCr水平及体重指数(BMI)均高于女性患者,血清球蛋白(GLB)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(APOA1)水平均低于女性患者,差异有统计学意义(P<0.05);不同性别患者年龄及HbA1c、胱抑素C(Cys-C)、ALB、总胆固醇(TC)、TG、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白B(APOB)水平和ACR比较差异无统计学意义。不同性别患者HbA1c与ALB、SUA、SCr均呈负相关(r<0,P<0.05),与TG均无相关性,ACR与ALB均呈负相关(r<0,P<0.05);男性患者ACR与TG呈正相关(r>0,P<0.05),与SUA、SCr无相关性;女性患者ACR与SUA呈正相关(r>0,P<0.05),与SCr、TG均无相关性。结论T2DM患者在控制血糖的同时可避免血清ALB的下降,男性T2DM患者在降低TG的同时可减少ACR,女性患者获益不明显。 Objective To analyze the correlation of glycated hemoglobin A1c(HbA1c),urine microalbumin/urine creatinine(ACR)with serum albumin(ALB),serum uric acid(SUA),serum creatinine(SCr)and triglyceride(TG)in patients with type 2 diabetes mellitus(T2DM)of different genders.Methods The clinical data of 597 patients with T2DM in Endocrinology department of Weishan People's Hospital from July 2020 to July 2021 were retrospectively analyzed,blood samples and urine samples were detected for all patients.The clinical data of T2DM patients of different genders were compared,Person correlation was used to analyze the correlation of HbA1c,ACR with ALB,SUA,SCr and TG in T2DM patients of dif-ferent genders.Results The levels of SUA,SCr and body mass index(BMI)in male T2DM patients were higher than those in female patients,while the levels of serum globulin(GLB),high density lipoprotein cholesterol(HDL-C)and apoprotein A1(APOA1)were lower than those in female pa-tients,and the differences were statistically significant(P<0.05);there were no significant differences in age,HbA1c,cystatin-C(Cys-C),ALB,total cholesterol(TC),TG,low density lipoprotein cholesterol(LDL-C),apolipoprotein B(APOB)levels and ACR between different genders.HbA1c was negatively correlated with ALB,SUA and SCr in patients of different genders(r<0,P<0.05),but had no correlation with TG,and ACR was negative-ly correlated with ALB(r<0,P<0.05);ACR was positively correlated with TG(r>0,P<0.05),but had no correlation with SUA and SCr in male pa-tients;ACR was positively correlated with SUA(r>0,P<0.05),but had no correlation with SCr and TG in female patients.Conclusion T2DM pa-tients can avoid the decrease of serum ALB while controlling blood glucose,male patients with T2DM can reduce ACR while lowering TG,but fe-male patients have no obvious benefit.
