黄芪建中汤结合雷火灸对脾胃虚寒型慢性胃炎的疗效及对T细胞亚群的影响

Effect of Huangqi Jianzhong Decoction Combined with Thunder-fire Moxibustion on Chronic Gastritis of Spleen and Stomach Deficiency Cold Type and T Cell Subsets

目的 探讨脾胃虚寒型慢性胃炎患者使用黄芪建中汤结合雷火灸治疗的效果。方法 将兴国县中医院2022年1月至2023年1月收治的脾胃虚寒型慢性胃炎患者86例使用随机数字表法分为两组,各43例,对照组实施四联抗幽门螺杆菌(Hp)治疗,基于此,观察组加用黄芪建中汤结合雷火灸治疗,均治疗2周,比较两组治疗效果、中医症候积分、T细胞亚群、血清指标及不良反应。结果 与对照组治疗总有效率76.74%比较,观察组治疗总有效率93.02%更高(P<0.05)。治疗后,两组中医症候积分均降低,且观察组更低(P<0.05);两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均提高,且观察组更高,两组CD8^(+)水平均降低,且观察组更低(P<0.05);两组血清胃蛋白酶原Ⅰ(PGⅠ)、胃泌素17(G-17)及胃蛋白酶原Ⅱ(PGⅡ)水平均提高,且观察组更高(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论 脾胃虚寒型慢性胃炎患者使用黄芪建中汤结合雷火灸治疗效果满意,可改善血清指标,提高患者免疫功能,且安全性好。

Objective To investigate the effect of huangqi jianzhong decoction combined with thunder-fire moxibustion on chronic gastritis of spleen and stomach deficiency cold type.Methods A total of 86 patients with chronic gastritis of spleen and stomach deficiency cold type treated in Xingguo Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were divided into 2 groups by random number table method,with 43 cases in each group.The control group received quadruple anti-Hp treatment,based on this,the observation group was treated with huangqi jianzhong decoction combined with thunder-fire moxibustion,for 2 weeks.The therapeutic effect,TCM syndrome score,T cell subsets,serum indexes and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.02%,higher than that of the control group(76.74%)(P<0.05).After treatment,the scores of TCM symptoms of both groups were decreased,and the observation group was lower(P<0.05);the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)were increased in both groups,and higher in the observation group,while CD8^(+)levels were decreased in both groups,and lower in the observation group(P<0.05);the serum levels of pepsinogenⅠ(PG-Ⅰ),gastrin 17(G-17)and pepsinogenⅡ(PG-Ⅱ)in 2 groups were increased,and those in observation group were higher(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion The effect of huangqi jianzhong decoction combined with thunder-fire moxibustion on chronic gastritis of spleen and stomach deficiency cold type is satisfactory,which can improve serum index and immune function of patients with good safety.
