IS合并SAS患者肺通气功能和糖脂代谢的临床研究

摘要 目的临床研究缺血性脑卒中(IS)合并睡眠呼吸暂停综合征(SAS)患者肺通气功能与糖脂代谢情况。方法收集吉林省神经精神病医院2022年1—10月收治的IS患者54例为观察组,并根据是否合并SAS,将其中30例单纯性IS患者设为观察1组,24例IS合并SAS患者设为观察2组,并选取同时间段健康体检者30例为对照组。回顾性分析所有研究对象的一般资料、肺通气指标、血氧饱和度、血脂情况及胰岛素抵抗(空腹胰岛素与胰岛素抵抗指数)指标。结果观察1、2组高血压、糖尿病和高血脂总数均多于对照组,肺通气指标与血氧饱和度均低于对照组,差异有统计学意义(P<0.05);与观察2组比较,观察1组肺通气指标与血氧饱和度更高,差异有统计学意义(P<0.05)。观察1、2组血脂指标均不在正常范围内,对照组血脂指标处于正常范围且低密度脂蛋白胆固醇、总胆固醇、甘油三酯指标均低于观察1、2组,高密度脂蛋白胆固醇高于观察1、2组,差异有统计学意义(P<0.05);与观察2组比较,观察1组各项血脂指标更优,差异有统计学意义(P<0.05)。观察1、2组空腹胰岛素与胰岛素抵抗指数均高于正常范围,对照组两项指标均处于正常范围且均低于观察1、2组,差异有统计学意义(P<0.05);与观察2组比较,观察1组空腹胰岛素与胰岛素抵抗指数更低,差异有统计学意义(P<0.05)。结论IS患者合并SAS,其肺通气功能降低,体内的糖脂代谢更差。 Objective To study pulmonary ventilation function and glucose and lipid metabolism in patients with ischemic stroke(IS)combined with sleep apnea syndrome(SAS).Methods A total of 54 patients with IS admitted to Jilin Neuropsychiatric Hospital from January to October 2022 were selected as the observation group.Depending on whether SAS was present,30 patients with simple IS were assigned to observation group 1,24 patients with IS combined with SAS were assigned to observation group 2,and 30 cases of healthy medical checkups in the same time period were selected as the control group.Retrospective analysis of general information,lung ventilation indicators,blood oxygen saturation,blood lipid levels,and insulin resistance(fasting insulin and insulin resistance index)indicators for all selected individuals were conducted.Results The total number of patients with hypertension,diabetes,and hyperlipidemia in the observation group 1 and 2 was higher than that in the control group,and the lung ventilation indicators and blood oxygen saturation were lower than those in the control group,with statistically significant differences(P<0.05).Compared with observation group 2,observation group 1 had higher lung ventilation index and blood oxygen saturation index,with statistically significant differences(P<0.05).The blood lipid indicators in the observation group 1 and 2 were not within the normal range,while the various indicators in the control group were within the normal range,and the low density lipoprotein cholesterol,total cholesterol,and triglyceride indicators were lower than those in the observation 1 and 2 group,and the high density lipoprotein cholesterol was higher than that in the observation group 1 and 2,with a statistically significant difference(P<0.05).Compared with observation group 2,observation group 1 had better blood lipid indicators,with statistically significant differences(P<0.05).The fasting insulin and insulin resistance index of the observation 1 and 2 group were both higher than the normal range,while the two indicators of the control group were both within the normal range and lower than the observation group 1 and 2 group,with statistically significant differences(P<0.05).Compared with observation group 2,observation group 1 had lower fasting insulin and insulin resistance index,with statistically significant differences(P<0.05).Conclusion IS patients with combined SAS have reduced pulmonary ventilation and worse glycolipid metabolism in the body.
