对老年慢阻肺患者使用千金苇茎汤加减联合常规西药治疗的效果分析

Analysis on the effect of modified Qianjin Weijian Decoction combined with conventional western medicine in the treatment of elderly patients with chronic obstructive pulmonary disease

目的探究对老年慢性阻塞性肺疾病(慢阻肺,COPD)患者使用千金苇茎汤加减联合常规西药治疗的效果。方法86例老年COPD患者,采用计算机随机分为对照组和观察组,每组43例。对照组实施常规西医治疗,观察组实施千金苇茎汤加减联合常规西药治疗。对比两组患者临床疗效、治疗后不良反应发生率及治疗前后肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC]、呼吸困难程度。结果观察组患者治疗总有效率为97.67%,明显高于对照组的83.72%(P<0.05)。治疗后,观察组FEV1、FVC、FEV1/FVC分别为(2.61±1.02)L、(3.05±0.57)L、(85.45±14.33)%,明显高于对照组的(1.94±0.95)L、(2.54±0.64)L、(76.45±13.28)%(P<0.05)。治疗后,两组患者COPD评估测试(CAT)评分均低于本组治疗前,且观察组患者CAT评分(7.69±2.42)分低于对照组的(15.62±4.53)分(P<0.05)。治疗后,观察组患者不良反应发生率6.98%显著低于对照组的23.26%(P<0.05)。结论通过对老年COPD患者在治疗期间实施千金苇茎汤加减联合常规西药治疗,能够有效提升治疗效果,缓解患者呼吸困难程度,改善患者的肺功能,降低不良反应发生率,可推广应用。

Objective To explore the effect of modified Qianjin Weijian Decoction combined with conventional western medicine in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD).Methods 86 elderly patients with COPD were randomly divided into a control group and an observation group,with 43 cases in each group.The control group received conventional Western medicine treatment,and the observation group received modified Qianjin Weijian Decoction combined with conventional Western medicine treatment.Patients in both groups were compared in terms of clinical efficacy,incidence of adverse reactions after treatment,pulmonary function indicators[forced expiratory volume in one second(FEV1),forced vital capacity(FVC),FEV1/FVC]and severity of dyspnea.Results The total effective rate of the observation group was 97.67%,which was significantly higher than 83.72% of the control group(P<0.05).After treatment,FEV1,FVC and FEV1/FVC in the observation group were(2.61±1.02)L,(3.05±0.57)L and(85.45±14.33)%,which were significantly higher than(1.94±0.95)L,(2.54±0.64)L and(76.45±13.28)%in the control group(P<0.05).After treatment,COPD assessment test(CAT)scores in both groups were lower than those before treatment,and the observation group had lower CAT score of(7.69±2.42)points than(15.62±4.53)points in the control group(P<0.05).After treatment,the incidence of adverse reactions in the observation group was 6.98%,which was significantly lower than 23.26% in the control group(P<0.05).Conclusion For elderly patients with COPD,modified Qianjin Weijian Decoction combined with conventional Western medicine can effectively improve the therapeutic effect,alleviate the severity of dyspnea,improve the lung function of patients,and reduce the incidence of adverse reactions,so it can be widely applied.
