多序列MRI检查诊断腰椎压缩性骨折的价值

Value of multi-sequence MRI in the diagnosis of lumbar compression fractures

目的观察多序列MRI检查诊断腰椎压缩性骨折的价值。方法回顾性收集2022年10月-2023年10月期间江西省乐平市中医医院收治的72例初诊高度疑似腰椎压缩性骨折患者临床资料,入院后均接受多层螺旋CT、多序列MRI检查及手术治疗,且临床资料及影像学检查资料完整。以手术结果为金标准,分析多层螺旋CT及多序列MRI诊断腰椎压缩性骨折的价值。结果该研究纳入的72例疑似患者中,经手术检查,确诊腰椎压缩性骨折65例(阳性),非腰椎压缩性骨折7例(阴性);多层螺旋CT检查54例阳性,与手术结果一致性较理想(Kappa=0.653);多序列MRI检查65例阳性,与手术结果一致性极好(Kappa=0.806);多序列MRI诊断腰椎压缩性骨折敏感度、准确度高于多层螺旋CT(P<0.05),且阳性预测值高于多层螺旋CT。结论多序列MRI检查诊断腰椎压缩性骨折敏感度较高,可为临床提供诊断及治疗依据,临床可推广应用。

Objective To observe the value of multi-sequence MRI in the diagnosis of lumbar compression fractures.Methods Clinical data of 72 newly diagnosed patients with highly suspected lumbar compression fracture admitted to the hospital from October 2022 to October 2023 were retrospectively collected.All patients received multi-slice spiral CT,multi-sequence MRI and surgical treatment after admission,and the clinical data and imaging data were complete.Using surgical results as the gold standard,the value of multi-slice spiral CT and multi-sequence MRI in diagnosing lumbar compression fractures was analyzed.Results Of 72 suspected patients included in this study,65 cases(positive)of lumbar compression fractures and 7 cases(negative)of non-lumbar compression fractures were confirmed by surgical examination.The results of multi-slice spiral CT were positive in 54 cases,which was in good agreement with the results of surgery(Kappa=0.653).The results of multi-sequence MRI were positive in 65 cases,which was in good agreement with the results of surgery(Kappa=0.806).The sensitivity,accuracy of multi-sequence MRI in diagnosing lumbar compression fractures were higher than those of multi-slice spiral CT(P<0.05),and positive predictive value also higher than multi-slice spiral CT.Conclusion Multi-sequence MRI has high sensitivity in the diagnosis of lumbar compression fracture,which can provide diagnosis and treatment basis for clinical diagnosis and clinical application.
