综合护理干预在剑突下电视胸腔镜纵隔肿瘤切除术患者围术期中的应用效果及对睡眠质量、心理状态的影响

Effects of comprehensive nursing intervention on sleep quality and psychological state in patients with subxiphoid video-assisted thoracoscopic mediastinal tumor resection during perioperative period

目的分析在为接受剑突下电视胸腔镜(Video-assisted thoracic surgery,VATS)纵隔肿瘤切除术患者实施治疗的过程中围术期综合护理干预的应用效果及对睡眠质量、心理状态的影响。方法此次试验研究的对象收治于2019年1月-2022年10月之间,共包括在天津市胸科医院接受剑突下VATS纵隔肿瘤切除术治疗的患者64例,根据随机数字表法将患者分为研究组与参照组,每组各有患者32例,其中研究组患者所运用的护理方法为围术期综合护理干预,参照组患者所运用的护理方法为围术期常规护理干预,对比分析两组患者的护理效果。结果研究组患者手术时间、术中出血量、留置胸腔引流管时间、下床时间与住院时间均显著差别于参照组患者,P<0.05。统计学分析两组患者护理前后睡眠质量、心理状态以及疼痛程度后可知,护理前两组患者并无显著差异,P>0.05;护理后研究组患者睡眠质量更好,心理状态较平稳且疼痛程度较为轻微,显著差别于参照组患者,P<0.05。研究组患者并发症发生率明显低于参照组患者,P<0.05。结论围术期综合护理干预是剑突下VATS纵隔肿瘤切除术患者的一种理想护理方法,值得临床推广。

Objective To analyze the effect of perioperative comprehensive nursing intervention on sleep quality and psychological status of patients undergoing Video-assisted thoracic surgery(VATS)for mediastinal tumor resection.Methods The subjects of this study were admitted from January 2019 to October 2022,including 64 patients who received mediastinal resection of subxiphoid VATS tumor in our hospital.The patients were divided into study group and reference group according to random number table method,with 32 patients in each group.The nursing method used by the study group was perioperative comprehensive nursing intervention,the nursing method used by the reference group was perioperative routine nursing intervention,and the nursing effect of the two groups of patients was compared and analyzed.Results The operative time,intraoperative blood loss,thoracic drainage time,getting out of bed time and hospitalization time of the study group were significantly different from those of the reference group,P<0.05.After statistical analysis of sleep quality,mental state and pain degree before and after nursing,there was no significant difference between the two groups of patients before nursing,P>0.05;after nursing,patients in the study group had better sleep quality,stable mental state and mild pain degree,which were significantly different from those in the reference group,P<0.05.The complication rate of the study group was significantly lower than that of the reference group,P<0.05.Conclusion Perioperative comprehensive nursing intervention is an ideal nursing method for patients with mediastinal tumor resection of subxiphoid VATS,and it is worth promoting in clinical practice.
